Service Contract
This Agreement between___________ ____________ (hereinafter referred to as “Client”) and Grey Matter Dog Training(hereinafter known as “Trainer”) pertains to the following:

Vaccination: Adult Dogs The owner must provide the trainer with proof that the dog has been vaccinated for rabies, Bordetella, and must be free of parasites (worms, fleas, ticks, mites, etc). Vaccination: Puppies The owner must provide the trainer with proof that the puppy has been vaccinated for distemper, hepatitis, leptospirosis, parvo, parainfluenza and must be parasite free before being allowed to be on the ground during training sessions outside the client’s home. Client acknowledges and understands the risks associated with owning and/or training a dog, including without limitation the risk of dog bites to Client or others, needing to rehome or career-change a dog not suited to work as a service/therapy dog, and any attendant emotional or physical distress or injury from raising and/or training a dog. Client knowingly and voluntarily accepts these risks by entering into this agreement. Sign Below To Agree
Cancellation Policy: If Client fails to give at least 24 hours cancellation notice, or is not present at time of scheduled appointment, session fees are still due. Sign Below To Agree
Payment: Fees must be paid when scheduled or at the start of the training session. Accepted payment methods are cash app, paypal, venmo, cash, and online website payments. Trade and barter may be accepted at the Trainer’s discretion. Payment must be made before services will take place. Sign Below To Agree
Travel Fees: Grey Matter Dog Training is located in Shawnee Oklahoma. Our service area includes up to 60 miles (or approximately 1 hour total travel time). We charge a travel fee of $15 for every additional 30 miles to those outside our service area. Sign Below To Agree
Services: Trainer agrees to provide private lessons for Client and Dog on a lesson-by-lesson basis, the goal being to teach Client how to train and work with Dog. These lessons will take place at Client's home. Trainer will make every reasonable effort to help Client achieve training and behavior modification goals but makes no guarantee of Dog’s performance or behavior as a result of providing professional animal behavior consultation. Client understands that he/she and members of the household must follow Trainer’s instructions without modification, work with dog daily as recommended, and constantly reinforce training being given to Dog. Failure to comply with the trainer's requirements via verbal or written statements, use of non-approved equipment, or non-compliance with the terms and conditions of the agreement may result in termination without refund.Sign Below To Agree
Liability: If Dog causes property damage, or bites or injures any dog, animal or person (including but not limited to Trainer and Trainer’s agents), during or after the term of this Agreement, then Client agrees to pay all resulting losses and damages suffered or incurred, and to defend and indemnify Trainer and Trainer’s agents from any resulting claims, demands, lawsuits, losses, costs or expenses, including attorney fees. If Dog is injured in a fight or in any other manner during or after the term of the Agreement, Client assumes the risk and agrees that Trainer should not be held responsible for any resulting injuries, losses, damages, costs or expenses. The parties confirm that, except for that which is specifically written in this Agreement, no promises, representations or oral understandings have been made with regard to Dog or anything else. Without limiting the generality of the foregoing, Client acknowledges that Trainer has not represented, promised, guaranteed or warranted that Dog will never bite, that Dog will not be dangerous or vicious in the future, that Dog will not exhibit other behavioral problems, or that the results of the training will last for any particular amount of time. This Agreement may be amended only by a written instrument signed by both Client and Trainer. Sign Below To Agree
Termination Grey Matter Dog Training may terminate this contract and stop services if the client violates state/ federal animal laws and or agreements made in this contract. Sign Below To Agree
Media Release Client understands and agrees that Client and Client’s dog may be videotaped or photographed during lessons or consultations to enhance Client’s training skills. Such photographs or videos, including Client’s and Client’s dog’s likeness, image, voice, appearance, and written communications, along with Client’s and Client’s dog’s name, may be used by At Your Service for website, social media, promotion, publicity, or other digital or written materials. If Client does NOT sign, Grey Matter Dog Training will still train, but will not use digital materials publicly.
Service Dogs While Grey Matter Dog Training endeavors to support Client in acquiring or training a dog to meet Client’s goals and preferences, Client recognizes that animal behavior is unpredictable, and animals may develop unforeseen health or temperament issues, or turn out to be unsuited to Client’s expectations or goals. Client understands and agrees that Grey Matter Dog Training offers no guarantees or express/implied warranties with regard to assessment, recommendations, training, and/or products or services, and that the same do not carry a guarantee of effectiveness with the particular dog and/or of meeting Client’s training, behavior, or working/service dog goals. Clients may be asked to provide a service dog prescription letter as proof of disability before service dog training takes place. Clients must understand that service dog training may take from 6-18 months or longer. During this time, the trainer may work with the client in their home, workplace, and/or other environments. Clients must not represent their dog as a service dog trained by Grey Matter Dog Training until their dog has passed all three Canine Good Citizen Tests or Grey Matter Dog Training’s Public Access and Task Training Test. Once the dog meets those requirements Grey Matter Dog Training can provide letters to assist with housing, flying, and workplace accommodations. Sign Below To Agree
Equipment Rentals Grey Matter Dog Training has books and gear with limited availability to be rented by paying clients. These items may be used by Grey Matter Dog Training but are the personal property of Meri Knowlton. Items may not be in perfect or like new condition when you rent them but should be returned in the same state they were rented. When using these items clients may rent them for up to 30 days. Strollers for socializing puppies may be rented longer than 30 days but must be returned once the dog is fully vaccinated. The client agrees to take liability of an item once they decide to rent it and will reimburse Grey Matter Dog Training for the full price to repair or replace the item if the item becomes damaged or destroyed.
