This form is a controlled document, managed by the NZHGPA delegation.

Notes:

1. This form must be filled out by the CSO; the completed form will be automatically returned to sender (CSO) and sent to NZHGPA Operations;
2. Fields marked * are required.
3. For authorisation procedures, refer to the OPM 2.3.3(F) Scope of Works, OPM 3.4.9 (E) CSO and OPM 8.4.2 Flying Site Authorisation;
4. Site Location: You must supply sufficient detail to identify all launch areas covered by this form;
5. Base the Safety Rating on the hazards documented & presuming use by Pilots unfamiliar with the site;
6. If training use is intended, note any variation to the site rating, when an Instructor is supervising;
7. A Site may have multiple launch and landing sites (i.e. Skyline Gondola has 3 approved launch sites and 2 approved landing sites)
