This form is a controlled document, managed by the NZHGPA, under the delegation of the Civil Aviation Authority (CAA).

CSO to fill: this form must be filled out by the CSO;
Fields marked * are required;
Authorisation procedures: refer to the OPM 2.3.3(F) Scope of Works, OPM 3.4.9 (E) CSO and OPM 8.4.2 Flying Site Authorisation;
Site Location: You must supply sufficient detail to identify all launch options covered by this form;
Safety Rating: base the safety rating on the hazards documented and presuming use by Pilots unfamiliar with the site;
Training use: if training use is intended, note any variation to the site rating, when an Instructor is supervising;
Multiple launch/landing options: One Site may have multiple launch and landing options (i.e. Skyline Gondola has 3 approved launch options and 2 approved landing options).

The below information is about your Club's MAIN flying sites; sites you maintain, publish and promote. These launch and landing areas are to be assessed for associated hazards, ratings, and relevant safety information disclosed. The NZHGPA, as per Part 149 requirement, will hold the central site authorisation data base. The NZHGPA may populate a list of authorised sites, those that the Club publishes and promotes, for pilot use. All disclosed PRIVATE launch/landing information will remain confidential.*
