I understand that boarding my pet(s) with Hamilton Plaza Animal Hospital gives the veterinarians and staff permission to bring my pet up to date on vaccinations, testing, parasite prevention and control, and treat for any unforeseen illnesses or injuries that may occur. Initial here I also understand that if I choose to leave a bed, blanket, toy, etc., I do at my own risk and that the staff will not give them to my pet(s) if they impose any hazards such as choking or potential ingestion of harmful materials. This includes leashes, collars, toys, etc. Food will be placed in a container with pet's name, if not already done so by the owner. If you feed an unregulated pet food, such as table scraps, meat, people food, etc, Hamilton Plaza Animal Hospital reserves the right to not feed due to possibility of illness. Initial here Medications will be kept in our treatment area and must be in original container with original instructions clearly visible.All pet's will be walked a short distance to the outside pens, while on a leash. If you prefer we DO NOT walk your pet(s) outside to the pens please Initial here Dogs are walked 2-3 times per day, fed twice a day, and have their water refreshed/filled as needed. You can purchase an additional package at $10/day that allows extra time outside, another walk, and a daily treat. On the days we are closed, Sunday and all major holidays, dogs are walked twice a day and 3 times if the package is purchased.For cats, they are attended to twice per day; scooping and/or cleaning the litter box, fed and fresh water. We do not walk cats. Cats remain in their kennel condos the duration of their boarding.I understand my pet must be up to date on required boarding vaccines and a yearly fecal check for intestinal parasites. All pets must be flea and tick free and on prescription flea/tick prevention. If any external (fleas or ticks) or internal (such as round, hook, tape worms, etc.) parasites are noted, Hamilton Plaza Animal Hospital will treat as needed at my expense. Initial here Medications will be administered to pets for an additional charge. The charge is per day, not per night. The cost is $4.00/day for medications and $6.00/day for diabetic patients. If any medications need to be refilled that cost will be added to your bill.Any pets that need to be isolated from other pets due to medical conditions will be charged an Isolation Fee of an additional $20/night.Boarding charges are for each night your pet(s) stays with us and is determined by weight. All pets must be picked up no later than 5:30 during the week, Monday-Friday, and by noon on Saturday. We are closed on Sundays and all major holidays and have no drop offs or pick ups available on those days.All pets not picked up within 7 days after the expected date of pickup, will be considered abandoned and become property of Hamilton Plaza Animal Hospital. This does not relieve me of the financial obligation from the pet(s) boarding charges. By initialing and signing this form you agree to not hold Hamilton Plaza Animal Hospital or it staff responsible for conditions that are often unavoidable in a boarding and/or hospital environment. These include, but not limited to, weight loss, diarrhea, kennel cough, upper respiratory infections, and stress. Some pets become nervous in a kennel setting and can show medical signs of this as well.
