All goats are registered therapy animals. They are likely to interact, which includes jumping on you and your back while doing various yoga poses at any given time. Please use caution and common sense. Goats have hooves, (may potentially have) horns, teeth, and adorable waggy tails. It is solely your responsibility to watch out for yourself and/or your child. Please be aware of the potential to interact with pet danders, goat urine and/or feces, and goat saliva.
GOOD PHYSICAL HEALTH:
I represent and warrant that I am in good physical health and do not suffer from any medical condition which would limit my participation in the classes offered at Life Repair Center of Front Royal. I understand that it is my responsibility to consult with a physician prior to and regarding my participation in any of the yoga classes, programs, or workshops. I understand the risks associated with the activities offered by Life Repair Center of Front Royal, and I agree to follow all instructions so that I may safely participate in classes, workshops, or other activities. It is my responsibility to know my own limitations and use common sense for myself and/or my child.
ASSUMPTION OF RISK:
I recognize that there is an element of risk of injury participating in any Life Repair Center events or activities, including the potential for severe physical injury, permanent disability and even death, and while particular rules, instruction, and personal discipline may reduce the risk, the risk of serious injury to me does exist.

I have read this Release of Liability, fully understand its terms, understand that I have given up substantial rights by signing it, either for myself or for persons under the age of 18 to whom I am a parent or legal guardian, and sign it freely and voluntarily without any inducement.
