This form is a controlled document, managed by the NZHGPA, under the delegation of the Civil Aviation Authority (CAA).

1. Instructors & Speedwing Coaches must use this form to assess sites suitable for their operation (OPM 8.4.2 (3) Training Sites);
2. Fields marked * are required;
3. Authorisation procedures: refer to the OPM 2.3.3(F) Scope of Works, OPM 3.4.9 (E) CSO and OPM 8.4.2 Flying Site Authorisation;
4. Site Location: You must supply sufficient detail to identify all launch areas covered by this form;
5. Training use: if training use is intended, note any variation to the site rating, when an Instructor is supervising;
6. Multiple launch/landing options: One Site may have multiple launch and landing options (i.e. Skyline Gondola has 3 approved launch options and 2 approved landing options).
