TO BE PAID OUT LATER, PLEASE BE READY WITH THE FOLLOWING. CFA no longer collects bank details or tax forms on this form or by email. When your project requests its first disbursement (or whenever your bank account changes) you will receive an invitation from Melio, CFA's payments provider, to enter your own payment details securely. Have ready: (1) a bank account in the project's or project representative's name (name must match your fiscal sponsorship agreement); (2) your routing and account numbers, or SWIFT/IBAN for international wires; (3) a signed W-9 (or W-8BEN for non-US payees), which Melio will request from you; (4) a current CFA balance report, dated within four weeks, when you request funds. Never send bank or tax information by email.
