Six Week Course Financial Agreement Form 

Please keep a copy of this form for your personal records.

 TIAWOM Equipping Center Course Payment Options: 

• Pay In Full: $90.00 per each 6 week Courses (Deliverance 101, Prophetic 101, Apostle 101, etc.
• Weekly Installments: $15.00 each week. The first week payment is due upon Registration of classes. The remainding payments are to be paid the Friday prior to Class.    
• A one time $25 Registration Fee is due at the time of registration.     
• NOTE: 12 Week "Ordination Track" is available upon request

All information provided in this form is stricly confidential. Upon signing this agreement students understand and agree to follow the plan of agreement selected and to pay in a timely manner. If the student selects weekly payments they must pay prior to the next class in order to attend that session. If there is going to be a delay or a problem with payment the President, Apostle Desmon Dobbins, ask that the student contact the school and Place the Course Name and Financial Agreement Concern in the subject line.
