  • By agreeing to the placement of permanent jewelry by 'til death by LUNA I understand and agree to the following terms and conditions:

     


    1. **Procedure Consent:** I consent to the permanent jewelry installation procedure performed by a trained and qualified professional at 'til death by LUNA. I have been informed about the process, including any associated risks and aftercare instructions.


    2. **Risk Acknowledgment:** I acknowledge the risks associated with permanent jewelry, including but not limited to infection, allergic reaction, skin irritation, and discomfort. I accept full responsibility for any complications that may arise during or after the procedure and waive any claims against 'til death by LUNA, its employees, and affiliates.

     
    3. **Age Confirmation:** I confirm that I am of legal age (18 years or older) to receive permanent jewelry or have obtained parental or guardian consent if under the legal age.

     

    4. **Health Disclosure:** I have disclosed any medical conditions, allergies, or medications that could affect my ability to safely receive and heal from the permanent jewelry procedure. I understand that failing to disclose this information can increase my risk of complications.

     
    5. **Wear and Tear Acknowledgment:** I understand that all jewelry is subject to wear and tear over time, and the longevity and appearance of my permanent jewelry may be affected by external factors including, but not limited to, physical activities, exposure to chemicals, and general care. I agree that 'til death by LUNA is not responsible for the repair or replacement of my jewelry due to normal wear and tear or my negligence in care.

     
    6. **Quality and Investment Clause:** I acknowledge that selecting higher quality jewelry can be an investment that may render better durability and resistance to wear and tear. I understand that investing in higher quality materials for my permanent jewelry can enhance its longevity and appearance over time. I agree that the selection of jewelry quality is my responsibility and that 'til death by LUNA has provided me with sufficient information to make an informed decision.

     

     7. **Aftercare and Maintenance:** I agree to follow all aftercare instructions provided by 'til death by LUNA to ensure the best outcome for my permanent jewelry. I understand that failure to adhere to these instructions may negatively affect the overall appearance of my jewelry and life span.

    •Avoid excessive exposure to lotions, perfumes, and chemicals
    •Jewelry can be washed gently with a soft toothbrush and dish soap
    •To remove, cut the jump ring with scissors, and don't lose the chain!


    8. **Liability Waiver:** I hereby release, waive, discharge, and covenant not to sue 'til death by LUNA, its officers, employees, agents, and affiliates from any and all liability, claims, demands, actions, and causes of action whatsoever arising out of or related to any loss, damage, injury, including death, that may be sustained by me or to any property belonging to me, while receiving or after receiving permanent jewelry.
