  • It’s time to register for Band and Guard for
    the 2024-2025 school year!
     
    Parts 1 and 2 of Registration are required for ​all students​ doing an extracurricular band activity in the 2024-2025 school year:
    • Marching Band and Color Guard
    • Winter Percussion (auditions in late Fall)
    • Winter Guard (auditions in late Fall)
  •  Part 1 of Marching Band/Guard Registration is a set of district and departmental forms:
    • FUHSD Parent-Guardian Field Trip Permission and Medical Authorization (Minor)
    • FUHSD Athletic-Activity Participation Authorization and Emergency Form
    • FUHSD Parent-Student Agreement Form
    • HHS Instrumental Music Behavior Expectations and Attire Guidelines
    • HHS Music Department Photo, Video & Interview Release Form

    Note:​ These forms require the student’s insurance information, student’s medication information, and a primary phone number for an emergency contact who is not the student's parent or guardian. 

    Part 1 of Marching Band/Guard Registration must be completed for each student participant by Friday, July 12th for the Fall 2024 season. The Part 1 Registration deadline for the Winter 2025 season will be announced in Fall 2024.

    Note: Students who are also taking an instrumental music class (e.g. Wind Ensemble 1 or 2, Symphonic Band, Concert Band, or Jazz Ensemble) in the 2024-2025 school year will be registered for their music class by completing Part 1 of the Marching Band and Color Guard registration.

  • Part 2 of Marching Band/Color Guard Registration is the FUHSD Physical Exam Form (Pages 1 and 2):

    • FUHSD Physical Exam Form - Page 1 - Student-Athlete Health History completed by a parent/guardian
    • FUHSD Physical Exam Form - Page 2 - Physical Exam completed by the student's medical provider

    FUHSD policy is that physical exams are good for one year and must not expire at any point during the season in which the student is participating (Fall or Winter). 

    Physical exam forms dated in 2023 will NOT be accepted
    for the Fall 2024 Marching Band/Color Guard season.
     
    A current physical exam form must be on file before a student can
    participate in Marching Band/Color Guard activities.

    INFORMED CONSENT & SPORTS INJURY RISK WARNING STATEMENT


    By its very nature, competitive athletics can put students in situations in which serious, catastrophic, and perhaps fatal accidents could occur. By granting permission to your son/daughter to participate in athletic competition, a parent/guardian acknowledges that playing or practicing in any sport can be a dangerous activity involving MANY RISKS OF INJURY. Both the athlete and parent must understand that the dangers and risks of playing or practicing include but are not limited to: death, complete or partial paralysis, brain damage, serious injury to virtually all internal organs, bones, joints, ligaments, muscles, tendons, and other aspects of the skeletal system and potential impairment to other aspects of the body, general health and well-being. However, it is impossible to totally eliminate such incidents from occurring.

     

    Because of the dangers of participating in sports, players may reduce the chance of injury by following coaches' instructions regarding playing techniques, training, equipment and other team rules, etc., both in competition and practice and agree to obey such instructions. Damaged equipment must be reported immediately. Even if all these requirements are met and even if the athlete is using excellent protective equipment, we understand that a serious, maybe even a fatal accident may occur.

     

    INSURANCE REQUIREMENTS (Parents must also complete this section)

    California law (Ed. Code Section 32221) requires every member of any interscholastic athletic team, as well as those associated directly with any interscholastic team, athletic event, including song and cheerleaders, team mascots, team manager, etc. to possess accidental bodily insurance providing at least $1,500 of scheduled medical and hospital benefits.   • The success of the Homestead Music Department relies on support from families and friends. You can help our award-winning programs through volunteering in various ways throughout the year. Volunteer opportunities include instrument check-out/check-in, fitting uniforms for students, selling and/or serving food at home football games, loading/pushing equipment, leading an activity, plus many more.

     

    We ask you to help our students during competitions, football games, and/or performances. Please indicate below what areas are of interest to you.     NOTE: Physical exam forms are good for one calendar year from the date they are signed, and must be valid through the end of the competition season.

     

    Forms dated in 2022 will not be accepted for the 2023-2024 school year. If your student needs an updated form, please allow enough time to schedule a physical exam before pre-season activities start.

     

    Students without a current Physical Exam Form on file will not be allowed to participate in Marching Band, Color Guard, Winter Percussion or Winter Guard.

     

    Soft copies of signed Physical Exam Forms can be submitted at this link. Once your Physical Exam Form is on file, Part 1 of your registration will be complete.
