Thank you for your interest in a CalAgPlate to show your support for agricultural education! Use the form below to request a sequential CalAgPlate for an automobile registered to an individual (not a business). Before you start, you will need a picture of your DMV vehicle registration.
If renewal of your vehicle registration is required within the next 60 days, please renew your registration prior to submitting your CalAgPlate Specialty License Plate Form.
If this is a new vehicle, please submit the application after you receive the permanent registration card.
To request a CalAgPlate for an automobile registered to a business, motorcycle, or trailer please print and mail a paper application to CA FFA, CalAgPlate, PO Box 460, Galt, CA 95632. DMV will not accept emailed or copied applications.

General:
- The license plate ordered will be assigned to a vehicle in my possession.
- The name & signature on this form must match the name on the vehicle registration.
- Once a license plate is purchased (ordered), your request can not be canceled.
Annual Renewal Fees:
The California Association, FFA will pay the initial plate fee. Owners are responsible for the Annual Plate Renewal Fee at the time of renewal. If the plate is not on a registered vehicle, you must pay an Annual Retention Fee to retain your interest in the license plate.
