Standard Waiver Release Form

This Agreement waives the liability of Next Level Sports Performance for any use of the services and facilities at TASIS England, included but not limited to the tennis courts, school grounds, and sports centre (gymnasium).

A NON-PARENT CANNOT LEGALLY SIGN THIS WAIVER FOR OTHER PEOPLE’S CHILDREN. IT MUST BE SIGNED BY A PARENT/LEGAL GUARDIAN.

PLEASE READ CAREFULLY AND SIGN BELOW.

1. I, the applicant signing below wish my child/children to utilize the services provided by Next Level Sports Performance and facilities at TASIS England, included but not limited to the tennis courts, school grounds, and sports centre (gymnasium).

2. I hereby agree that the use of the services provided by Next Level Sports Performance and facilities at TASIS England, included but not limited to the tennis courts, school grounds, and sports centre (gymnasium), is at my, my spouses, or guardian's and my children’s own risk. As a condition of my, my spouse, and my children’s use of such services provided by Next Level Sports Performance and facilities at TASIS England, included but not limited to the tennis courts, school grounds, and sports centre (gymnasium), I, on behalf of myself, my heirs and assigns and on behalf of my children’s (and my children’s heirs and assigns) expressly agree to forever discharge, waive and release Next Level Sports Performance, its directors, management, staff, servants, agents, employees and/or independent contractors and their heirs, successors and assigns from any and all claims, demands, injuries, liabilities, actions, causes of action and from all acts of active or passive negligence on the part of Next Level Sports Performance and its directors, management, staff, servants, agents, employees and/or independent contractors and their heirs, successors and assigns on account of any and all injuries or damages, including but not limited to bodily injury, mental injury and/or property damage from any event, mishap, accident, loss, damage or injury suffered by myself or my child resulting from or connected with or caused by the use of the services provided by Next Level Sports Performance and facilities at TASIS England, included but not limited to the tennis courts, school grounds, and sports centre (gymnasium). I further agree to defend, indemnify and hold harmless Next Level Sports Performance and its directors, management, staff, servants, agents, employees and/or independent contractors and their heirs, successors and assigns from any and all claims, losses and liabilities arising from, connected to, and/or arising from my, my spouses, and/or my children’s use of the services provided by Next Level Sports Performance and facilities at TASIS England, included but not limited to the tennis courts, school grounds, and sports centre (gymnasium).

3. I declare and affirm that my child/children are in good medical and physical condition, and can participate in the services provided by Next Level Sports Performance and facilities at TASIS England. I further agree to abide by the rules and regulations of Next Level Sports Performance, as they now exist or may be amended in the discretion of Next Level Sports Performance.

I have read and understand the above and foregoing and acknowledge my consent to terms of this Waiver and Release for myself and my child.

Photo and Video Disclaimer

From time to time, pictures and video's may be taken of participants involved with Next Level Sports Performance.

We would like to able to use these photographs and videos for marketing materials (flyers), company social media account and our website. Names will not be posted unless written authorization is given by the student and parent/guardian, and then only first names will be used. If there are concerns about pictures or videos posted on the website, please contact us, and they will promptly be removed.

Photos and/or videos taken by Next Level Sports Performance Limted will be stored inline with our data protection policy and only used with the consent of the parent (if child is under 16) or child.
