Waiver

I give full consent and authority for the Player to attend the AUSA Hoops AAU Tour Trials and other related AUSA Hoops events, and I accept full responsibility for them.

I am aware that signing this form affects my legal responsibilities.

In consideration of payment for participating in the activity (and except to the extent that the same may be precluded by statute) agree to release and indemnify AUSA Hoops and its Operators, Owners, Staff, Contractors and all others related to the AUSA Hoops operation as follows;

1. The Player participates in the activities in the Venue at their and my, own risk and responsibility.
2. I release and hold harmless AUSA Hoops, its servants and agents, from and against all and any actions which may be made by me or on my behalf or by other parties for or in respect of, or arising out of injury, loss, damage or death caused by or to the Player, the Players or my property whether by negligence, breach of contract or in any way whatsoever.
3. I accept all responsibility for any injury, loss or illness that may arise from the Player’s participation in the AAU Trials and related events. I will not hold AUSA Hoops responsible in any way.
4. I also agree that in the event that I or any children in my custody or on the premises of the gym are injured or my property is damaged, I will bring no legal claim or otherwise against AUSA Hoops, its servants and agents in respect of that injury or damage.
5. I grant permission to AUSA Hoops and it’s partners and affiliates, for the rights of my image, likeness and sound of my voice as recorded on audio or video tape without payment or any other consideration for content that is captured at our around the AUSA Hoops AAU Trials or other related events.  I understand that my image may be edited, copied, exhibited, published or distributed and waive the right to inspect or approve the finished product wherein my likeness appears. Additionally, I waive any right to royalties or other compensation arising or related to the use of my image or recording.  I also understand that this material may be used in diverse educational settings within an unrestricted geographic area. I grant permission for the photographic, audio or video content to be utilised in AUSA Hoops and rebel digital, in-store or other channels. There is no time limit on the validity of this release nor is there any geographic limitation on where these materials may be distributed.
