  • Lifetime Member Gift

    This form will ask for basic payment logistics from each person sponsoring the membership- please just submit one form per sponsored Lifetime Member. Only submit it once you have a pledge totaling $1000 for Professional Lifetime Membership and $800 for Associate Lifetime Membership. If you have more than 5 sponsors, please reach out to membership@shpe.org for instructions on how to submit this form. The first installment must be paid for the person receiving the LM gift to receive their pin onstage during the Lifetime Member Reception during National Convention.
