Fairytale Spa - Liability Waiver

Fairytale Spa team would like to thank you for allowing us to participate in your party celebration. We hope to make all your sparkling dreams a Reality!

I, the Participant or Parent/Guardian, desire to take part in the Spa party by Fairytale Spa.

The services provided at Fairytale Spa are for ENTERTAINMENT PURPOSES ONLY and though all staff are trained professionals they are not licensed cosmetologists.

PARTY POLICIES

• We do not use any sharp utensils to perform manicures and pedicures.
• We do not cut nails.
• We do not use electricity with water, we only use warm water basins.
• We use disposable makeup applicators for each guest at each party.
• Spa robes, Costumes and Party Props are the property of Fairytale Spa and are not intended for guests to keep. Unless your Party Package and contract Indicate that.
• It is your responsibility to notify the staff of Faitytale Spa if your child has any allergy.
• We take no responsibilities for any allergic reactions of any party guest. It is the responsibility of the parent or guardian hosting the party to alert Fairytale Spa in advance of any allergies that any child might have.
• Fairytale Spa reserves the right to refuse services on any child, for any reason that we may feel may impose a health risk to others.
• Fairytale Spa takes no responsibility for any injuries or property damage caused by any party guest. It is the responsibility of the parent or guardian hosting the party to supervise the children keeping them safe and under control during the party.
• In case of lost or damage of any Fairytale Spa equipment or party items caused by a party guest whether intentional or unintentional, the parent or guardian hosting the party will be held responsible for the monetary reimbursement of that item.
• Each party attendees will require a signed permission waiver by their parents or guardian prior
  
  

I affirm that I have read this agreement in its entirety and that I understand the nature of the spa services that will be provided to my child. I agree that my child and I will hold harmless Fairytale Spa and its staff for personal injuries and property damage arising out of or related to unintentional or negligent conduct of the service provider and hereby waive all claims suits, losses or related causes of action arising from such conduct for damages including but not limited to, such claims that may result from injury during or arising in any way from the spa services.

(We do not share E-mail addresses. We will occasionally send information, specials and coupons)

Photos or videos takes by Fairytale Spa are the sole ownership of Fairytale Spa and we may use them for promotional purposes on our website, print ads or publications. We always respect our client's privacy and for this reason, your personal information will not be disclosed to third parties.

I acknowledge that photographs or images of myself/my child may be taken during the activity.
