History of Treatment

It is extremely important to provide all of your treatment for this condition since you were discharged from service. In some circumstances, your statement alone could be enough to link your condition to service. This information is crucial to your case.

Please list ALL hospitals or medical facilities where you have treated since you were discharged from service. This includes VA Medical Centers and civilian facilities or outside providers. 

Be sure to provide at least the YEAR you were FIRST seen at each facility. This is important to ensure the VA obtains all potentially relevant medical records for your case. If you're not sure what month you began treating there, put 01/01/yyyy.

Note: If you have been to the same medical facility multiple times, you only need to list the facility's name once.
