  • ANY PERSON WHO IS NOT A MEMBER OF LOCAL 181 AND WISHES TO WORK IN OUR JURISDICTION MAY REGISTER FOR WORK. 

    REGISTRANT MUST COMPLETE A SKILLS QUALIFICATION FORM, AND ATTACH CERTIFICATIONS, IF ANY, AND SUBMIT ALL PAPERWORK. 

    REGISTRATION IS $19 PER MONTH AND THE REPORT-OUT­ DATE WILL BE THE DATE THE FEE IS RECEIVED. THE REPORT­ OUT-DATE CAN BE RETAINED BY RE-REGISTERING THE FIRST WEEK (BY THE 7TH OF THE FOLLOWING MONTH), OTHERWISE A NEW REPORT-OUT-DATE WILL BE ASSIGNED. WE WILL ACCEPT ONLY ONE MONTH'S REGISTRATION FEE PER MONTH (NO ADVANCE PAYMENTS). 

    IF A REGISTRANT IS REFERRED TO WORK, THEY SHALL BE REQUIRED TO PAY SERVICE DUES. SERVICE DUES FOR NON­ MEMBERS ARE $10 PER WEEK. SERVICE DUES FOR MEMBERS OF ANOTHER IUOE LOCAL ARE $5 PER WEEK.

    AFTER IUOE 181 STAFF VERIFY ALL FORMS, PAYMENTS CAN BE MADE VIA CHECK, CASH OR CREDIT CARD.  IF PAYING BY CREDIT CARD, PLEASE CONTACT YOUR DESIGNATED PRIMARY DISTRICT OFFICE THE DAY AFTER SUBMISSION TO COMPLETE THE PAYMENT PROCESS.

  • As a member of another IUOE Local, you have the option to request a transfer of your Health and Welfare Contributions at the end of this application for your convenience. Please fill out the Request for Health and Welfare Reciprocity if needed.

  • I hereby designate the INTERNATIONAL UNION OF OPERATING ENGINEERS and its subordinate Local Union No. 181 to represent me for the purpose of collective bargaining and in any and all other situation that may arise under the operation of the National Labor Relations Act, and/or with any individual employer where the provisions of the Labor Relations Act are not invoked.

  • TO: All Employers who are members of the Construction Employers Association of Central Kentucky, Inc.; A.G.C. of Indiana; Highway Contractors, Inc.; Indiana Constructors, Inc.; Tri-State Contractors Association, lnc.; West Kentucky Construction Employers Association, Inc.; all other Employers who are Signatory to a Collective Bargaining Agreement with International Union of Operating Engineers, Local 181 and its Subordinate Branches; and to all lnternational Agreement Signatories performing work in the jurisdiction of Local Union 181.

     

    I hereby voluntarily assign to the International Union of Operating Engineers Local 181 and its Subordinate Branches and authorize and direct that each of you deduct from my gross wages earned or to be earned by me, as your employee (in my present or in any future employment by any of you), the Sum of 4% (four percent) for each hour of gross wages paid me. I authorize and direct each of you to remit the same to the union at such times and in such manner as may be agreed upon between the Union and the Signatory Employers at any time while this authorization is in effect.

    This assignment and authorization shall remain in full force and effect until revoked in writing by me but is irrevocable while employed by any of the above Signatory Employers. Such a notice revoking this assignment and authorization shall be given by written notice delivered by certified mail to the Union and to the Employer with whom I was last employed.

  • KY PAC of International Union of Operating Engineers Local 181 Deduction Authorization

  • I hereby authorize my employer to deduct from my wages the sum of $0.10. per hour for each hour worked and to remit such amount to the Kentucky PAC of the International Union of Operating Engineers, Local 181, P.O. Box 34,Henderson, Kentucky 42419.

    THIS AUTHORIZATION IS VOLUNTARILY MADE BASED ON MY SPECIFIC UNDERSTANDING THAT: The signing of this authorization card and the making of contributions to Kentucky PAC of the International Union of Operating Engineers, Local 181 are not conditions of membership in the union nor of employment with my employer and that I may refuse to do so without fear of reprisal I further recognize I may withdraw this authorization at any time upon written notice to my employer.

    I am making a contribution to a fund-raising effort sponsored by Kentucky PAC of the International Union of Operating Engineers,Local 181, and acknowledge and wish for the Kentucky PAC of the International Union of Operating Engineers, Local 181 to use my contributions for political purposes, including but not limited to, the making of contributions to or expenditures on behalf of candidates for federal, state, and local offices and addressing political issues of public importance in Kentucky and Indiana, as well as for all other lawful purposes in the Kentucky PAC of the International Union of Operating Engineers, Local 181's interest.

    Federal law requires us to use our best efforts to collect and report the name, mailing address, occupation, and the name of employer of individuals whose contributions exceed $200 in a calendar year. Contributions or gifts to Kentucky PAC of the International Union of Operating Engineers, Local 181 are not deductible as charitable contributions for federal income tax purposes.

  • IUOE/Engineers Political Education Committee (EPEC) Deduction

    Contributions or gifts to the lUOE EPEC are not deducible as charitable contributions for Federal income tax purposes. A copy of lUOE EPEC's report is filed with the Federal Election Commission (FEC) and is available from the FEC in Washington, DC

  • I hereby authorize and direct that each employer signatory to an agreement with the International Union of Operating Engineers or any of its affiliates for whom I work to deduct from my paycheck five cents ($.05) per hour or  ($             ) per hour for all compensated hours as an employee, and to remit such amount to the IUOE Engineers Political Education Committee (EPEC), 1125 17™ St. NW, Washington, D.C 20036 at such times as other remittances are made to the union.

    This contribution is voluntarily made upon the specific understanding that the making of such voluntary contributions is not a condition of membership in the Union or of employment with the employer; that I may contribute a greater or lesser amount than that suggested and I will not be favored or disadvantaged for doing so, that I may refuse to contribute without reprisal, and that IUOE EPEC uses the money it receives to make political contributions and expenditures

    This authorization shall remain in full force and effect until revoked in writing by me.

  • Health & Welfare Reciprocity

    Request and Authorization for Transfer of Contributions

  • I request and authorize the Board of Trustees of the Local 181 Health and Welfare Fund to transfer the Health and Welfare Fund contributions made on my behalf to the Fund I have listed below. This authorization will be effective on the date it is received and within six months prior to that date. This authorization will remain in effect until this authorization is revoked in writing. In support of this request, I state as follows:

     

       1. I am a member of IUOE Local No. {whatUnion} and my Union Register No. is  {registrationNum}

  •   2. I understand that, upon approval of my request to transfer, I cannot later request that any contributions which may be transferred to my Horne Fund be transferred back to the transferring Fund.

      3. I understand that, upon approval of my request to transfer contributions, my (and my dependents') eligibility for benefits and all other participant rights shall be determined exclusively by the terms of my Home Fund's plan and rules, and not by the terms of the transferring Fund's plan and rules.

      4. By making this request, I waive and release, on behalf of myself and my Dependents, any and all claims against both Funds and their fiduciaries relating to whether the transfer of contributions Is in my or their best interests.
