Waiver of Liability:

By submitting this form, I acknowledge the risk in physical activity during dance, fitness, and movement classes and release Dirigo Conservatory of Movement from all liability of injury that may occur while participating in said risk.
Payment is due at the start of the first class and can be made in the form of credit card, cash, check to Dirigo Consveratory of Movement, or Venmo @dirigoconservatoryofmovement. Annual Registration Fee $25 per family, due at the time of registration.

*Prices listed are monthly. Pay in full for the 9 month school year by September 1st, 2024 and recieve a 10% discount. Or split into two installments due September 1 and January 1 for 5% off. All-Styles program students also have the option to perform in the Year End Performance. This is a storyline based production of 1-1.5 hours long, costumes are rented from our Costume Collection for a small fee.
