Statement of Consent & Waiver

I hereby agree and declare that I am the legal parentguardian of the above-named couselor and hereby consent to the counselor's participation in the activities that are described to me in registration process. I understand that activities of the kind described may result in physical injury to my child but nonetheless specifically request that he or she be allowed to participate in those activities.
If the above-named child requires any emergency medical treatment or procedures during the activities, I hereby consent to activity supervisor(s) to make any decision and take any action to arrange for such procedures or treatments in the discretion of the activity supervisor(s).
I, the parent/guardian, hereby agree and declare that I have carefully read and understand the scope of the summer camp activities and I consent to the participation of the above-named child to these activities. I provide consent for pictures to be taken of my child for publication of camp experiences. I hereby agree to take any responsiblity for any damage or crashed that should ever occur while transporting campers or staff members, and exempt Camp Shlomo or any of its staff, of any responsibilities regarding any type of damage or cost. I consent for my child/ or myself, to drive Camp Shlomo campers or any children who got a ride with my child.

I provide consent for my child to be driven by staff to any/all Camp Experiences. I hereby release and fully forever discharge Camp Shlomo, the Jewish School of Miami (The Jewish Academy), Temle Solel, and all of its administrators, teachers and staff, supervisors, and agents, from liabilities, claims, demands, suits and causes of action of every kind in any way relating to or arising out of my childs/childrens participation in the camp or all its activites.
