I authorize "Unity Christian Food Service" hereinafter called "Company," to initiate debit entries to my account indicated below and the Financial Institution name below, hereinafter called "Financial Institution," to debit the same account. I acknowledge that the origination of ACH transactions to my account must comply with U.S. law.

ACH payments will not be established until September 2024 and will run through May 2025 unless otherwise instructed via email to bforeman@unitychristian.net.
Please upload a oicture of a deposit slip or voided check for proof of account ownership. If you participated in prior years you do not need to submit a new one, unless the account has changed.

This authorization is to remain in full force and effect until Company has received written notification from me (or any authorized account signer) of its termination in such time and manner as to afford the Company a reasonable opportunity to act on the request. Unless prior authorization to terminate this request is made, the Company will terminate the request on 5/31/2025.
