LOCAL 598 APPRENTICESHIP APPLICATION INSTRUCTIONS
1) Complete this application form in its entirety.
- Submit proof of valid driver’s license
- Submit a copy of a document that shows proof of age: birth certificate, passport, baptismal certificate, etc. A driver’s license is NOT acceptable.
- Submit official high school transcripts and copy of diploma. Certified equivalent scores (GED) may be used in lieu of HS transcripts. Also include official college transcripts and/or trade school certifications, if applicable
- Military Discharge papers (DD-214), if applicable. If active duty, letter from commander must be submitted.
- (3) Personal and (3) Professional letters of recommendation. (6 Letters Total)
2) You will be notified via email after your application has been reviewed and approved. You will then have 60 days to complete your drug test and bring it to the training center office. Drug test submissions may be brought in on Fridays between 9-12 or 1-4. At that time you will review your submitted application and sign it for submission. You must bring the following items:
- Valid Driver’s License
- Non-D.O.T. drug test at Lourdes Occupational Health Facility at own expense approximate cost is $50. The results are mailed directly to Local 598
3) After successful and complete submission of your application you will be directed via email to complete the following steps:
- Complete the Online Journey as instructed
- Complete proctored math assessment scheduled and proctored at Local 598 Training Center
- Appear for interview at schedule time
Notes:
You must be 18 years of age by the time of the interview
NO exceptions can be made for information submitted late.
If you are missing any of the above information, your file will be considered INCOMPLETE and will not be accepted. It MUST be completed to be accepted.
