Initial here*I acknowledge that behavior change cannot be guaranteed. Behavior modification cannot always yield exact results, and eliminating or changing target behaviors is not always possible.

Initial here*I understand that my cat's behavior change cannot be assured in any set period of time, as each cat is different due to ability, breed, temperament, and learning experience.

Initial here*I consent for Blissful Cat Behavior Consulting to contact my cat's veterinarian for medical history and vaccination status.

Initial here*I consent for Blissful Cat Behavior Consulting to discuss my cat's medical/behavioral history, medications, previous appointments, or other pertinent information with my veterinarian or other relevant animal professionals.

Initial here*I agree to accept responsibility for any damages or injuries my cat may cause through any behavior that may occur before, during, or after all services provided by Blissful Cat Behavior Consulting. I will not hold Blissful Cat Behavior Consulting responsible in the event of injury to people or other animals.

Initial here*I understand that cats participating in training sessions are free of any infectious disease and are current on all appropriate vaccinations, including FVRCP and rabies.

Initial here*I understand Blissful Cat Behavior Consulting operates on a policy of mutual respect. Any verbally abusive language or actions directed toward Luke in any context will not be tolerated, and those responsible will be asked to seek services elsewhere.

Initial here*I understand that I am required to be on time for my appointments. If I do not show up for my session without notice, I will be considered a no show and will not be refunded. If I am more than 10 minutes late for my session, I will be considered a no show and will not be refunded

Initial here*Withstanding emergency situations, appointments may only be rescheduled with 24 hours notice. Failure to adhere to this policy will result in a $25 rescheduling fee.
