Holiday Activities Registration (March 2025)

Cost: CAD 200/Week or CAD 45/Day

Early Bird price with 10% discount

Age Group and Time:

- Ages 4 to 12: 9 am to 12:30 pm

What to Bring:

- Child will bring their own lunch/snacks (if kids require) and a refillable water bottle. Color dish, pencil, and eraser.

- Child must be able to use the washroom independently.

- Child should wear comfortable and weather-appropriate clothing.

- Please inform us of any medical conditions/allergies.

Activities Include:

- Bollywood Dance Basics (everyday), Art (one of the activities from following every day).

Art Activities:

- Painting, drawing, paper craft, clay art.

- Craft paper, painting materials, and clay will be provided.

At the end of the week, Kids coming for full week will be able to perform on the latest Bollywood song!!

Informed Consent and Acknowledgement

I hereby give my approval and conscent by submitting this form for my child’s participation in any and all activities prepared by Shreya Bollywood dance during the selected camp.  

By submitting this form
1. I/we realize that participation in dance classes and activities could result in some possible personal injury if steps are not performed correctly. Despite precautions being taken by the studio and team, accidents and injuries may occur. By signing this release form, I/we (the dancer and parent/guardian) assume all risks related to the use of any and all spaces used by Shreya Bollywood Dance.

2. I/we will not hold Shreya Bollywood Dance including all teachers, dancers, staff members, volunteers and facilities used by both entities, liable for any personal injury including: scrapes, bruises, cuts, sprains, fractures, broken bones, concussions or death or any personal property damage/loss, which may occur on the premises before, during or after classes or in any event where he/she goes for performance with Shreya Bollywood Dance.

3. I/we will not sue Shreya Bollywood Dance and Yugali Bharote from any and all liabilities or claims made as a result of participation in this activity, whether caused by the negligence of release or otherwise.

4. The final decision pertaining to batch activities allocation on daily basis, song selection for dance will be at the sole discretion of Shreya Bollywood Dance.

Media Disclosure Shreya Bollywood Dance including all teachers, dancers, staff members, volunteers and facilities used by both entities may periodically photograph students participating and working during camp. These photographs may be used in a variety of ways; Public relations and marketing purposes: images printed on brochures, posters, and newsletters, or on Shreya Bollywood Dance Website, Facebook Page, and/or other art, education, or news related publications. Please indicate whether you will allow publication of photographs taken of your child/children and their literacy work during classes. Names will not be included in photographs or posted on social media. We appreciate your support and would love the chance to share pictures of all of the fun being had at our Summer Holidays Activities. You can also follow us on Facebook to capture all of the pictures yourself!   

 

Students are not enrolled until full payment is received.

For Daily Selection, We will review the form with days selected and send final confirmation to you regarding spot status.
