  • Use the log below to track your dog's response to sounds. Each row represents one sound response. 

    • Practice: Check if this was a response to a practice sound. Leave blank if a real-life sound occured.
    • Alert Behavior: Select the behavior the dog used to alert you. If the dog did not alert, or you had to prompt the dog, use "Missed alert." 
    • Source sound: Choose what sound occured.
    • Distance to sound: Record your estimate of how far away from the sound source you were, in feet.
    • Handler location: Specify where you were when the sound occured, e.g. kitchen, garage, workplace, park, etc.
    • Handler activity: Describe what you were doing when the sound occured, e.g. washing dishes, reading, sleeping, etc.
    • Shown to source: Record how quickly the dog showed you to the source of the sound, if they were successful. 

    Use the "Notes" section to record your other observations. For example, were you or the dog distracted? What were the distractions? If the dog offered a false alert, how did you respond?

  • Use the log below to track your dog's response to low blood glucose samples or actual readings. Each row represents one response. 

    • Sample: Check if this was a response to a practice sample. Leave blank if a real-life low occured.
    • Blood Glucose: Record the sample number used or the number from your glucose meter.
    • Alert Behavior: Select the behavior the dog used to alert you. If the dog did not alert, or you had to prompt the dog, use "Missed alert." 
    • Handler location: Specify where you were when the low or practice occured, e.g. kitchen, garage, workplace, park, etc.
    • Handler activity: Describe what you were doing when the low or practice occured, e.g. washing dishes, reading, sleeping, etc.
    • False Alert: Check if your glucose reading was normal after an alert. Set your timer and recheck in 20 minutes. 
    • 20 min recheck reading: Note the glucose reading after 20 minutes. 

    Use the "Notes" section to record your other observations. For example, were you or the dog distracted? What were the distractions? If the dog offered a false alert, how did you respond?
