Dear Valued Patients, We hope this letter finds you in good health. As part of our commitment to providing accessible and comprehensive healthcare services. We will be offering a membership fee structure for our telehealth services. This membership fee will cover a range of benefits designed to enhance your access to quality healthcare from the comfort of your own home. We believe this model will provide greater convenience and affordability for our patients while ensuring continuity of care.

Key points regarding our membership fees: There is a 6 month commitment requirement

Monthly Subscription: Telehealth services are included as part of your existing DPC (direct primary care) membership fee will be structured as a monthly subscription, which will grant you access to virtual consultations, eligible prescription refills, sick visits, and follow-up care. This does not include visits for Weight management or Hormone therapy.

*One Time Membership Enrollment fee: $125

Monthly membership (Age 18-49): $75

Monthly Membership (Age 50+): $110

Sick visits, medication refills, Annual physicals, Specialty Referrals, Pre- authorizaions, Sleep Study referral, In office visit, telehealth visits, in office testing for (strep/flu) Access to Provider via email & text, as well as after hour/weekend access to provider via text for urgent needs
message & chronic care management (ex: HTN, DM) included in monthly membership.

DPC Member Urgent care option- VIP Membership – $140/month

Includes After-Hours Urgent Care access at no additional cost, plus 2 emergent concierge home visit per month.
Please note: The concierge visit must be within our designated service area (mileage restrictions apply—please inquire for eligibility based on your location).

Bundled Care Discount

If you are currently enrolled in Direct Primary Care (DPC) or Hormone Replacement Therapy (HRT), you may be eligible for a discounted rate on your monthly membership fee.

Patients participating in Weight Management who add HRT or DPC services may qualify for a discount on the DPC membership, as Weight Management is not currently structured as a membership-based program.

We value our long-term patients and are happy to offer customized pricing for those actively participating in more than one service.

Transparent Pricing: We are committed to transparency in our pricing. The DPC membership fee is per month, with no hidden costs, additional fees are based other services available. You will have the opportunity to review and approve the membership fee before enrolling in our telehealth program.

Flexibility: You will have the flexibility to cancel your membership at any time with proper notice to our practice. We understand that circumstances may change, and we want to ensure that our services are tailored to meet your evolving healthcare needs.

Insurance Coverage: Please note that while our DPC membership fee covers a comprehensive range of telehealth services, it does not replace health insurance. You may still use your health insurance for services not covered under our membership agreement, such as in- person visits with specialists, hospital care, laboratory.

I understand that enrollment includes a six-month minimum commitment. I acknowledge the required six-month participation period associated with this program. I agree to the initial six-month commitment as part of membership enrollment. I am aware that cancellation is not permitted within the first six months of membership.

WERE YOUR LABS COMPLETED WITHIN 6 MONTHS (CBC, CMP, TSH, A1C)?

Ready to start your weight loss journey today and know that you qualify? Book this initial consult. We will conduct a review of your medical history, labs, and weight loss goals. Then, a customized diet and exercise plan will be developed and tailored to your needs. If you have completed labs, you will need to provide in your first visit.

Prescription Refills- Our streamlined process ensures you have emergency access to the chronic medications you need, when you need them, with a 30-day supply until you are able to follow-up with your primary care provider. Labs should be up to date.

Please note: Controlled medications CANNOT be refilled. Refills must come from the original provider.

Enroll in our direct primary care program or chronic care management program for long-term care.
