By submitting this form, I am officially registering for Pick-Up Soccer with Community First Sports Club (CFS) and am agreeing to all of the following items to be known as the ("Terms and Conditions"):
- I have read, reviewed with my family, and accept CFS "Club Overview" and "General Policies & Procedures" located on the club website at www.communityfirstsoccer.com
- I have read, reviewed with my family, and accept CFS "Assumption of Risk and Liability Waiver with Regards to Coronavirus/COVID-19" and "COVID-19 Practice Guidelines" located on the club website at www.communityfirstsoccer.com
- CFS reserves the right to take video and photography of all players in practice and game setting to utilize for online media platforms, and for advertising, marketing, and other promotional material.
Pick-Up Soccer Requirements:
- All players that are not a part of CFS must complete and submit this registration form to take part in Pick-Up Soccer
- Players should arrive 15 - 30 minutes early for check-in and a light warm up.
- Players should arrive to Pick-Up Soccer wearing appropriate soccer attire, appropriate footwear, and shin-guards. Players not in appropriate attire will not be able to participate in Pick-Up Soccer
Parents will be receive a communication each week with updates regarding each weekend's Pick-Up Soccer event.
