    To Be Read and Signed by the Participant’s Parent/Guardian
  • I FULLY UNDERSTAND THAT: 

    (a) USING WOODWORKING TOOLS AND EQUIPMENT AND/OR SOLVENTS AND WOOD FINISHES INVOLVE RISKS AND DANGERS OF SERIOUS BODILY INJURY, INCLUDING PERMANENT DISABILITY, AND DEATH ("RISKS"); 

    (b) these Risks and dangers may be caused by the participant’s actions or inaction, the actions or inaction of others participating in the Activity, the condition in which the Activity takes place, or THE NEGLIGENCE OF THE "RELEASEES" NAMED BELOW; 

    (c) there may be OTHER RISK AND SOCIAL AND ECONOMIC LOSSES either not known to me or not readily foreseeable at this time; 

    and I FULLY ACCEPT AND ASSUME ALL SUCH RISKS AND ALL RESPONSIBILITY FOR LOSSES, COSTS, AND DAMAGES incurred as a result of the minor  in the Activity.

  • I THE MINOR'S PARENT AND/OR LEGAL GUARDIAN, UNDERSTAND THE NATURE OF INTERSECTIONS' ACTIVITIES AND THE MINOR'S EXPERIENCE AND CAPABILITIES AND BELIEVE THE MINOR TO BE QUALIFIED, IN GOOD HEALTH, AND IN PROPER PHYSICAL AND MENTAL CONDITION TO PARTICIPATE IN SUCH ACTIVITY. I HEREBY RELEASE, DISCHARGE, COVENANT NOT TO SUE, AND AGREE TO INDEMNIFY AND SAVE AND HOLD HARMLESS EACH OF THE RELEASEE'S FROM ALL LIABILITY CLAIMS, DEMANDS, LOSSES, OR DAMAGES ON THE MINOR'S ACCOUNT CAUSED OR ALLEGED TO BE CAUSED IN WHOLE OR IN PART BY THE NEGLIGENCE OF THE "RELEASEES" OR OTHERWISE, INCLUDING NEGLIGENT RESCUE OPERATION AND FURTHER AGREE THAT IF, DESPITE THIS RELEASE, I, THE MINOR, OR ANYONE ON THE MINOR'S BEHALF MAKES A CLAIM AGAINST ANY OF THE RELEASEES NAMED ABOVE, I WILL INDEMNIFY, SAVE, AND HOLD HARMLESS EACH OF THE RELEASEES FROM ANY LITIGATION EXPENSES, ATTORNEY FEES, LOSS LIABILITY, DAMAGE, OR COST ANY MAY INCUR AS THE RESULT OF ANY SUCH CLAIM.

  • I AUTHORIZE that in the event of an accident or illness affecting the minor indicated on this form, I authorize all procedures, including the admission to the hospital and necessary treatment herein as deemed essential for the care and well-being of said minor. Such action is to be taken only when immediate contact with the undersigned or other indicated authorized contacted cannot be made

  • I HEREBY RELEASE, DISCHARGE, AND COVENANT NOT TO SUE IWC and their respective administrators, Directors, agents, officers, members, volunteers, and employees, other participants, any sponsors, advertisers, instructor, teachers and staff and, if applicable, owner and lessors of premises on which the Activity takes place, (each considered one of the "RELEASEES" herein); {1) FROM ALL LIABILITY, CLAIMS, DEMANDS, LOSSES, OR DAMAGES ON MY ACCOUNT CAUSED OR ALLEGED TO BE CAUSED IN WHOLE OR IN PART BY THE NEGLIGENCE OF THE "RELEASEES" OR OTHERWISE, INCLUDING NEGLIGENT RESCUE OPERATIONS; (2) I FURTHER UNDERSTAND AND AGREE THAT ANY OR ALL PROPERTY (PERSONAL EFFECTS, TOOLS, CREATIVE SUPPLIES, ETC.) THAT I CHOOSE TO BRING ONTO THE PROPERTY AND LEAVE OR STORE AT THE PROPERTY IS DONE SO SOLELY AT MY OWN RISK AND THAT IWC WILL UNDER NO CIRCUMSTANCES BE RESPONSIBLE FOR ANY LOSS OR DAMAGE TO MY PROPERTY; {3) AND I FURTHER AGREE that if, despite this RELEASE AND WAIVER OF LIABILITY, ASSUMPTION OF RISK, AND INDEMNITY AGREEMENT I, or anyone on my behalf, makes a claim against any of the Releases, I WILL INDEMNIFY, SAVE, AND HOLD HARMLESS EACH OF THE RELEASEES from any litigation expenses, attorney fees, loss, liability, damage, or cost which may incur as the result of such claim.

  • I understand that if the participant withdraws or cancels within 5 business days of the first day of class no refund will be issued. If the participant cannot complete the program or cannot attend due to illness or other absence no refund will be issued for the days missed.

  • I HAVE READ THIS AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT AND HAVE SIGNED IT FREELY AND WITHOUT INDUCEMENT OR ASSURANCE OF ANY NATURE AND INTEND IT TO BE A COMPLETE AND UNCONDITIONAL RELEASE OF ALL LIABILITY TO THE GREATEST EXTENT ALLOWED BY LAW. I FURTHER AGREE THAT IF ANY PORTION OF THIS AGREEMENT IS HELD TO BE INVALID THAT THE REMAINDER SHALL CONTINUE IN FULL FORCE AND EFFECT.

  • I, , Parent/Guardian of   , grant Intersections Wood Collaborative my permission to take photos and/or videos of my participating child during their woodworking sessions at Intersections Wood Collaborative for any legal use, including but not limited to publicity, copyright purposes, illustration and web content.     

  • PERMISSION TO LEAVE THE PREMISES

  • The summer program may include events off site (for example, an outing to Harrison park to learn about forestry). Youth will travel by car with an adult who is affiliated with our organization. If parents/guardians are not comfortable with their youth leaving our facilities, we will have one adult who will stay back and supervise.

  • REFUND POLICY

    In order to be eligible for a refund, you must cancel your order five business days prior to the commencement of the scheduled youth session by contacting our program facilitator, madison@intersectionswoodcollabroative.org


    After we receive your cancellation, the money will be refunded to the original payment method you've used during the purchase. For credit card payments it may take 5 to 10 business days for a refund to show up on your credit card statement. 


    If you have requested a cancellation within 5 business days prior to the start date of the youth’s session, you will not be eligible for a refund. If anything is unclear or you have any questions feel free to contact our program facilitator, Abby. 

     

    CODE OF CONDUCT


    All students, instructors, staff and volunteers have the right to a positive and safe experience. Intersections Wood Collaborative follows these 3 core values: 


    Respect yourself: Being creative is important in fostering self-esteem, self confidence and self-respect. It is important to stay safe and healthy following the instructions and safety protocols given by instructors. 


    Respect others: Treat others like you would like to be treated, including fellow students and their families or support instructors, staff and volunteers. Equity, Diversity and Inclusion are shown through positive display of respect around speech, action, and behaviour. Verbal or physical abuse, violence, harassment, racism, intimidation and discrimination of any kind will not be tolerated. 


    Respect the environment: Our studio, equipment, materials, and building hold exciting and valuable equipment that are available to our whole community and need to be treated with respect, care and safety by always following directions given by the instructor.
