Generation Dance Registration Form

Classes Begin September 15!

Please read through all Class Descriptions, Class Fees/Tuition, and Handbook for Season 15 available on our website before registering. We reserve the right to decline the class selection if the student does not fit age/ability requirements. One form per student. Registration fee online is $40/family (returning) $50/family (new) and is only complete when the square payment is received. Weekly emails will be sent out the week prior to classes starting. If you are not receiving emails weekly, please reach out to stay informed!

Payment/Registration

Registration fee is due along with the registration form. First payment is due the first week of class (September 15-18). All other payments are due the first week of the month. Registration fee is non-refundable. See GDC HANDBOOK or our website for class prices. Full season payment is also an option.

*Monthly payment is due the first class of each month.

*All returned checks will incur a $25 fee.

*We reserve the right to refuse entrance to anyone (student or guardians) due to safety concerns, inappropriate behavior, or lack of payment.

*Missed classes and tuition payments are non-refundable.

*Costume payment of $70/class is due November 20, 2025

Liability Disclaimer: I for myself and/or minor child release and forever discharge and agree to hold harmless Generation Dance Company for any liability, claims or demands of personal injuries, sickness, or death, as well as for expenses due to loss or damage to personal property or lost wages. Since dance is a physical activity, injuries may occur. Each student/guardian has the right to decline participation in any activity which they are not comfortable with or which they feel may be harmful. Each student/guardian is fully responsible for any injuries or harm occurring before, during or aftera class at Generation Dance Company. Student/guardian is also responsible for informing instructor(s) of any physical limitation which may prevent full participation in class. If injury occurs, it is understood that the student's own insurance policy is the only source of reimbursement. Additionally, Generation Dance Company has permission to use photos/videos taken of the signed student for adverstising, sales of media from dance shows and for any promotional purpose.
