- The majority of our volunteer opportunities take place between the hours of 9am-4pm Monday-Friday and Saturdays May through October.

- Any volunteers age 16 or younger must be accompanied by guardian/supervisor*

The information collected in this Volunteer Interest Form will be used solely for the purpose of volunteer coordination, screening, and communication. Your data will not be shared, sold, or used for any purposes beyond those related to our volunteer programs.

By submitting this Volunteer Interest Form, you acknowledge and agree to the following:

Volunteer Commitment – Completing this form does not guarantee placement in a volunteer role. All volunteers must complete an orientation as necessary for specific roles.
Code of Conduct – Volunteers are expected to uphold respectful, inclusive, and professional behavior while participating in our programs. Any misconduct may result in removal from volunteer activities.
Liability Waiver – Volunteers understand and accept any risks associated with participation in our programs and agree to release Foothills Food Hub and LFAC from liability for any injuries or incidents that may occur while volunteering.
Confidentiality – Volunteers may have access to sensitive information, including participant details, and agree to keep all such information confidential.
Media Consent – Volunteers may be photographed or recorded during events for promotional use. If you prefer not to be included, please inform us in writing before participating.
