Hair Extension Waiver of Agreement

I, First Name and Last Name hereby acknowledge and agree to the following terms and conditions regarding my hair extension installation and maintenance services:

1. Deposit and Booking:
I understand that  a deposit is required at the time of booking my hair extension installation appointment. To secure my appointment with Beauty By Tanica, I acknowledge that Beauty By Tanica  requires a deposit of 50% of the total service cost. 
• I understand that my service deposit 
will go toward my day of service and is non refundable. This deposit will be applied toward my hair extensions as well as any other services I may have booked for that day. I agree that I am over 18, or that I have had a legal guardian review ,sign, and submit this form on my behalf. This deposit is nonrefundable if I cancel the appointment. However, I am allowed to reschedule the appointment once, and the deposit will be applied toward the final cost of the extension installation.
2. Finality of Service:
• I acknowledge that the hair extension service is final after the application. Any desired changes to the style after the application will incur additional charges.
 3. Hair Extension Care and Maintenance:
• I acknowledge that hair extensions are sensitive and different from natural, intact human hair.
• I understand that proper care and maintenance are crucial to prevent damage to both the extensions and my natural hair.
• I have been informed of and understand the necessary maintenance procedures, including the daily maintenance required to keep the extensions in optimal condition.
4. Protection of Natural Hair:
• I acknowledge that improper care of hair extensions can lead to damage to my natural hair. I commit to following the provided guidelines to ensure both the extensions and my natural hair remain healthy and undamaged.
5. Removal of Hair Extensions:
• I understand that the charge for the removal of hair extensions is not included in the original installation fee and will incur an additional cost.
6. Take-Home Hair Care Kit:
• I will be provided with a take-home hair care kit when new hair is purchased and a new installation appointment is made. The take-home hair care kit may include shampoo, conditioner, hair oil, heat protectant spray, and a wet brush.
7. Pricing Acknowledgment:
• I acknowledge that the price of my hair extensions has been discussed and agreed upon with my stylist. I understand that hair extension prices can vary depending on the client’s individual needs and the desired results.
• I understand that any additional services provided during the same appointment, but not related to hair extensions, will incur separate charges.
• I understand that any additional services provided during the same appointment, EXAMPLE:hair color services, custom-colored extensions, coloring natural hair, cutting natural hair, any chemical services to the natural hair, and/or cuttings hair extensions will incur separate charges ( with the exception of my my first hair extension cut which is included any time I purchase new hair to be installed as a new installment appointment). I am aware that any additional haircuts in following appointments will be charged as an extension haircut.
8. Storage Policy for Purchased Hair Extensions:
• If I purchase hair extensions and decide not to proceed with the installation service, Beauty By Tanica will securely hold my hair extensions for a period of 42 days from the date of purchase. I acknowledge that it is my responsibility to pick up my hair extensions within this period. Due to limited storage space, it is not feasible for your stylist (Tanica) to store extensions indefinitely. Therefore, if the extensions are not picked up within 42 days, they will be considered abandoned and will become the property of Beauty By Tanica.
9. Understanding and Acceptance:
• I have read this waiver agreement in its entirety.
• I voluntarily accept the terms of this release by signing below and warrant that I fully understand its contents.


By signing this waiver agreement, I agree to adhere to the terms and conditions set forth above.
