Sharing Your Outdoor Mindset Story

Guidelines for Sharing

Below is an outline for sharing your experience. Use these questions to help tell your story in your own words. If this format for sharing your story does not work for you, please reach out to us! We can also share your story via a Zoom chat or call together.

• Questions to think about:
  • What is the diagnosis that brings you to Outdoor Mindset?
  • When was the diagnosis made?
  • How has the diagnosis impacted everyday life?
  • How has the diagnosis impacted access to the outdoors and/or connecting with people?
  • How did you find Outdoor Mindset?
  • How has the Outdoor Mindset community impacted your life?
  • Why does Outdoor Mindset's mission and work matter to you?

  • Share photos and/or video relevant to your story below. Bonus points for some photos being outside!

OUTDOOR MINDSET PHOTO + STORY RELEASE FORM

I grant permission to include any images, video clips, biographical information, and story quotations from which I may be identified in “Sharing My Story” submission to be published by Outdoor Mindset.

I grant Outdoor Mindset the right to use “Sharing My Story” submission:

• On Outdoor Mindset’s website
• On Outdoor Mindset’s social media (Facebook, Instagram, Twitter, Linkedin)
• In Outdoor Mindset’s email newsletters
• In promotional materials for the organization

I hereby grant Outdoor Mindset the irrevocable and unrestricted right and permission to copyright—in their own name and otherwise—or use, re-use, publish, or re-publish photographic images and video clips that in any way include myself.

I understand that I may be included in part or in whole in any and all media for illustration, promotion, art, editorial, trade, website, video, social media, advertising or any other purpose whatsoever. I also consent to the use of any printed or electronic material referencing me. I understand that no compensation will be paid.
