I, the owner of the pet named above hereby authorize BIYAYA ANIMAL CARE and to whoever Veterinarian they may designate to perform the spay/neuter surgery of my pet that is named in this form.
I understand that my pet will undergo anaesthesia and other medications needed to perform the surgery. In line with this, the operation presents some hazards, and that injury to and post-operative infection in, or death of, the animal may conceivably result. I understand and accept these risks to the animal. I withhold Biyaya Animal Care from any liabilities without proper medical examination and communication with our veterinary team.
BLOODTEST PRIOR SURGERY: I understand the animal will have to receive pre-operative bloodwork prior the surgery to assess if they are PHYSICALLY FIT. For pets aging two (2) years old below, CBC (Complete Blood Count) is required with an additional price of Php 550. For pets aging three (3) years old and above, CBC and Blood Chemistry is required at an additional price of Php 1,800.
I am allowing my pet to be neutered without vaccination, I am aware of the risks and complications involved. I understand the inherent risk of failing to maintain current vaccinations and waive all claims arising out of, or connected with, the performance of this procedure due to such failure.
FEMALE CATS AND DOGS: I understand that if my pet is PREGNANT, I consent Biyaya Animal Care to terminate the pregnancy with an additional charge of Php 500 (for cats) and Php 2000 (for dogs)
FEMALE CATS AND DOGS: I understand that if my pet has PYOMETRA, (pus in the uterus), which will only be known during the surgery, I concent Biyaya Animal Care to continue with the procedure with an additional charge of Php 3000 (for cats) and Php 5000 (for dogs)
MALE CATS: I understand that if my pet has UNDESCENDED TESTICLES, I consent Biyaya Animal Care to continue with the procedure with an additional charge of Php 1000 (cat inguinal) / Php 2500 (dog inguinal) and Php 2000 (cat abdominal) / Php 3500 (dog abdominal).
