  As the parent or legal guardian of the above-named participant, I hereby give my consent for Grape Creek Baptist Church Vacation Bible School volunteers to seek and authorize emergency medical care. This care may be given under whatever conditions are necessary for the well-being of my dependent. Reasonable attempts will be made to notify parent(s)/guardian(s) prior to authorizing any such emergency care.
  
  I fully understand that my child must abide by all rules governing conduct and safety while attending Vacation Bible School at Grape Creek Baptist Church.
  
  Additionally, I give permission for my child to be photographed during activities associated with Vacation Bible School at Grape Creek Baptist Church. I understand that said photographs may be used for the promotion of Vacation Bible School at Grape Creek Baptist Church and/or shared on social media. No names will be shared in association with said photographs.
