Please provide your vet reference, include name of clinic, location and phone number. Please note, if you have changed vets in the past 2 years please provide prior vet reference also. Vet reference is also needed if your dog has passed away or you have moved. We need at least 2 years of current vet history. This includes, us verifying with your vet that your dog is up to date on heartworm prevention, core vaccines, regular dental cleaning as your vet recommends, and is spayed/neutered.

Chapman’s Dachshund Rescue asks for a 20.00 non refundable adoption application donation. Chapman’s Dachshund Rescue is a non profit 501c3 organization. The application fee/ donation insures that the prospective adoption family is committed and ready to go forward with welcoming a new rescue dog in their home. This is in addition to the regular adoption fee. This does not insure that we will have a dog that is a a good fit for your situation or the type of dog you are looking for.
