Transport Contract

By completing this form, you agree to all terms listed.

• Pets must be 8 weeks before transport
• Pets older than 16 weeks should have valid rabies vaccination
• Driver has the right to refuse transport if animal shows signs of aggression and driver sees no way to safely complete the transport
• If any pets need emergency medical care, driver will use own discretion and seek out veterinarian care if possible. Person signing this document and arranging transport will be responsible for veterinary costs.
• We try our best to stay on track, but unforeseeable events can always occur. (weather, traffic, vehicle troubles, and more.) Please be patient.
• Be sure to provide feeding and medicine instructions, with enough for the trip.
• A total of 25% of the cost is due at booking to secure your spot on our schedule. The remaining balance is due by the time we hand off your pets at drop off time
• 50% of the deposit is refundable for cancellations made 5+ days before scheduled pickup. Cancellations less than 5 days before pickup will result in no refund. This is because it gets more difficult to fill your spot last minute. Nonrefundable fees can, however, be applied as a credit toward the balance of a future transport(one time.)
• We will make every reasonable attempt to contact at drop off time. If we are unable to make contact/drop off, a fee of $100 per day will be added to balance to defray the cost of boarding.
• Cat bites, in particular, can be dangerous. Cats are also more of a flight risk. Because of this, it's important for pet parents to place cats in their carriers or kennels. Driver is not obligated to chase or load cats at pickup time.
• Paws On Wheels Pet Transport will not be held responsible for injuries, illnesses, etc.
