You are required to fill out this form so you can set an appointment for taking of entrance exam and initial screening of documents at the Registrar’s office. Successful submission of this registration form is not tantamount to official enrolment. You have to proceed to succeeding steps indicated in your email. Please follow strictly the scheduled appointment in order to observe social distancing in the campus.
