Trial Class Registration & Waiver

This form must be completed for all trial students. The trial class fee is $50. Once we receive your completed form, we will issue an invoice. Please note that payment must be made prior to the trial, either online or in person at the Tanglin Arts Studio office.

Please be aware that the trial fee is non-refundable in the event of cancellation.

Acknowledgement of Risk & Emergency Authorisation

I understand that my child will be participating in a physically active program, which carries an inherent risk of injury. In the event of an emergency, I hereby authorise Tanglin Arts Studio Pte Ltd to obtain necessary medical treatment for my child on my behalf.

I also acknowledge and agree that Tanglin Arts Studio Pte Ltd will not be held liable for any injuries, loss, or damage to personal property that may occur during participation in the program.
