LVLUP Birthday Party Waiver
Thank you for choosing LVLUP to celebrate your child's special day! We hope that you enjoy your time spent in the park with your friends.

There are a few things that you all need to adhere too whilst you are at LVLUP:

1) No parents / spectators other than the riders are allowed to be in the actual Skatepark

2) Riders must have helmets on at all times whilst they are in the actual Skatepark

3) Under NO CIRCUMSTANCES are any children allowed to slide down any of the ramps like they are slides. Our ramps have screws in them and can tear skin very quickly. If you want to go down slides, please go to an outdoor park!

4) No food and drink is to be taken into the actual Skatepark. There are areas outside with tables and a viewing area that accommdates for this.

5) No yelling, screaming or runnign around the Skatepark. There are other riders and people attending and we want to respect others that have paid to use ths park.

6) The outdoor ramps and airbag (whilst deflated) is out of bounds. These ramps are for pro athletes only and not to be used as slides or to run around on.

I am aware of the dangers of all participants attending LVLUP-AUS Training Facility. I promise not to hold LVLUP Group, its officers, agents, or employees liable or pursue legal action against them. This waiver absolves LVLUP Group of any duty for injuries sustained on the premises before to, during, or after the activity.
By signing this agreement, I agree to hold LVLUP Group completely harmless, including financial responsibility for any injuries sustained, regardless of the cause or circumstances.
I will ensure all staff and participants of my part obey all safety requirements, policies and proceedures of LVLUP and to seek clarification if necessary.
