  • Dog Re-Entry to the United States Questionnaire

    As of August 1st, 2025, there is a new CDC restriction on the importation of dogs from countries high-risk for dog rabies. Please answer the following questions for us to help determine how best to prepare you to return to the United States with your dog.

    Please fill out this form for each dog. The CDC rules apply to ALL dogs entering the United States, even if they originated from the United States.

    Please note, dogs may be correctly and appropriately vaccinated for rabies based on state law, but the rules for the CDC are different, and your dog may not be vaccinated appropriately per the CDC standards.

    If your dog is not in the United States, we are unable to provide assistance. You will need to contact a veterinarian in the country of export for guidance.

  • Requirements for import of a dog into the United States depend on where the dog has been. Dogs who have been in a country high-risk for dog rabies have additional requirements.

    Dogs re-entering the United States can re-enter using a Certificate of US Issued Rabies Vaccination (CUSIRV) which must be USDA endorsed prior to departing the United States.

  • As of 6/1/25, the following countries ARE HIGH RISK for dog rabies:

  • Please note, dogs under 6 months old are not allowed to enter the United States.

  • Please note, ALL dogs must be microchipped. The CDC will not recognize any rabies vaccinations prior to microchip placement.

  • Import from Countries Low-Risk for Dog Rabies:

    -Dogs must be healthy on arrival

    -Dogs must be 6 months or older

    -Dogs must be microchipped

    -Must have CDC Dog Import form completed 2-10 days before arrival

  • Import from Countries High-Risk for Dog Rabies:

    Requirements for ALL dogs:

  • The CDC has mandated that USDA Accredited veterinarians are ONLY allowed to provide Certification of U.S.-issued Rabies Vaccination (CUSIRV) forms for established patients with a valid Veterinary-Client-Patient Relationship (VCPR). If your dog has never been to Art City Vets, or has not been in for >1 year, you will need to make an in-person appointment to establish a valid VCPR.

  • Please note, a CUSIRV is a USDA-endorsed document and is different from a Rabies Vaccination Certificate.

    When veterinarians administer a Rabies vaccination, they must provide a Rabies Vaccination Certificate.

    To get a CUSIRV, a USDA Accredited veterinarian must submit Rabies Vaccination Certificate information to the USDA for endorsement. The cost through Art City Vets is $221 including the USDA endorsement fee. For rabies vaccinations we have adminsitered, the cost is $160.
