Employee Information (Full SS#s if working off old payroll reports). Employee #, Name, Address, SS#, Filing Status, Hire Date, Wage Pay Per Period, Dept. #, etc.)
YTD EE payroll (gross payroll to net payroll for each employee from January 1st with full tax dtail). Include terminated employees in the YTD information.
Copies of client quarterly 941 reports for the current year.
Quest Login and Password information.
Reporting Agent Authorization (form 8655) form signed.

List of Pay Types for Employees.
