    Use this application to express interest in being part of the Pennsylvania Homecare Associations' (PHA) Direct Care Worker Track of the Training and Mentorship Program funded by the Department of Labor and Industry Job Quality Grant.
  • Eligibility Requirements:

    • Applicants must be seeking employment or newly employed in home care, home health or hospice
    • Applicants should be new to the industry (within 6 months)
    • Applicants must work 40 hours in the industry to receive grant funding 
    • Applicants must work in Pennsylvania

    Steps For Grant Funding: 

    Step 1: Complete this application to apply to be considered for the grant program

    Step 2: Complete the MyLearning Center (MLC) Training Videos within 30-days of completing this application. Track your progress within MLC or using this form.

    Step 3: Accept a job offer with a home care, home health or hospice agency and work your first forty (40) hours. See PHA Job Board for work opportunities if you're not already hired by an agency

    Step 4: Complete the Grant Satisfaction Survey following steps 1-3. 

    By successfully completing the above items, you will be awarded $300.00.

  • The Department of Labor and Industry is asking for participation in their intake questionnaire regarding this grant program.

    The purpose of the survey is to gather insights on your current experiences and perceptions of your job and employer. Your feedback will be valuable in assessing your satisfaction, as well as identifying strengths and potential areas for improvement in your workplace.
