This form can be started & saved for later. All information below will be for internal use only. This is a STATE-only CTSO. After filling out this form, please have your students complete the 2026-2027 Student Membership Form that will be emailed to you. THIS IS A REQUIRED STEP IN ENROLLMENT. The student form includes their personal emails and other pertinent information we will need. If no student selects leadership positions, the Educator is responsible to designate the leadership positions to their students. The student Leadership Lab, which will be held in October, will kick off the school year. Please watch your email for more details.
We are required by the state of Ohio to charge a membership fee for each student. The 2026-27 membership fee is $5.00 per student.
