I Accept the following Terms of Use (see details below)We need to purchase insurance for this event.I have my own insurance (Upload proof of insurance below)JANITORIAL SERVICES * Agree (The Lab) agrees to maintain daily janitorial service for The Lab in Chandler, and the customer agrees to pick up any trash accumulated during their sessions.INSURANCE * A copy of the organization’s (or private individual) certificate of liability insurance listing “The Lab in Chandler” as the additional insured may be required for each use of the facilities.INDEMNIFICATION * Client agrees to hold (The Lab), its owners and its staff, harmless against all liabilities and claims arising from customer or customer’s players use and to defend (The Lab), its owners and its staff, against any claims arising from such use.

Terms of Use: Liability Insurance: $80.00/day Proof of Insurance. I am uploading a copy of our policy to complete this registreation. HOURS OF USAGE Agreement will allow Customer the use of The Lab in Chandler on designated day and during designated hours, which may change upon written request from the Customer and agreement by (The Lab). Upon written notification from the Customer, of hours that will not be used, (The Lab) reserves the right to use those hours at the facility for other activities. Type of Activity: PAYMENT: The Customer agrees to pay: * $65 per hour per court for AM use $75 per hour per court for PM use after 4:00pm The Customer agrees to pay: Enter other amount, subject to approval. Customer is requesting use of (number of courts). Invoice will be provided by (The Lab) to (Customer) upon submitting of this usage agreement. (The Lab) offers basketball/volleyball courts, bathrooms, and parking lot. TERMS * Payments are final and non-refundable unless agreed to in writing by (The Lab). Should the (Customer) refuse to pay or not pay in full, the amount listed on the Invoice, (The Lab) will pursue the balance in its entirety through legal action. (Customer) agrees, by signing this agreement, to forfeit their right to contest any unpaid fees in court and agrees to pay all the legal fees associated should legal action be required. (The Lab) reserves the right to add additional fees for any damages to The Lab in Chandler created by (Customer) activities. All invoices are payable by check or credit card, Zelle, Cash App or PayPal. All check payments will be payable to The Lab in Chandler. GOVERNING LAW This contract is governed by Arizona law and any disputes shall be resolved in an Arizona court of proper jurisdiction. ACCEPTANCE Customer has read and acknowledges this Agreement in its entirety. Upon signing, Contract becomes fully binding.

More Rules

A) No firearms or weapons of any kind are permitted on the premises. Violators will be prosecuted.

B) We have a zero-tolerance policy for fighting. Anyone fighting will be banned from The LAB.

C) All parties in violation of the law will be prosecuted should police need to be summoned to the premises.

D) No live animals will be allowed on the premises at any time. This does not include service animals.

E) (The LAB) is not liable for any property left on the premises before and after the designated usage hours.

Members of your group should not be on the floor until your designated time. Respect the time of those before you.
No outside food allowed inside The Lab. Water and or Sport Drinks only.
Use of the upstairs area (restroom and weights) allowed only if part of your reservation.
Your guests and minor children are not allowed upstairs without supervision.
Pick up and discard any trash, water bottles, and remove any items of clothing and sports equipment.
Please use downstairs restroom only
Do not flush paper products in the bathroom toilet.

AT THE END OF YOUR RESERVATION *

Clean up any spills created by your group
Check for personal items possibly left behind as The Lab is not responsible.
If there is no user in after you, turn off the lights. Overhead light panels in the utility room
Leave The Lab as you found it.
