READ BEFORE SIGNING - In consideration of being allowed to participate in any way in THE COLLECTIVE youth basketball league, related events and activities, the undersigned acknowledges, appreciates, and agrees that: 1. The risks of injury and illness (ex: communicable diseases such as MRSA, influenza, and COVID-19) from the activities involved in this program are significant and potentially life-threatening, and while rules, equipment, and personal discipline may reduce these risks, the risks of serious injury and illness do exist; and, 2. I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASES or others, and assume full responsibility for my participation; and. 3. I willingly agree to comply with the stated and customary terms and conditions for participation. If, however, I observe any unusual significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the nearest official immediately; and, 4. I acknowledge that I am aware that there are risks to me of exposure to directly or indirectly arising out of, contributed to, by, or resulting from:• An outbreak of all communicable disease, including but not limited to, the virus "severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2)", which is responsible for Coronavirus Disease (COVID19) and/or any mutation or variation thereof; 5. I acknowledge that I am fully responsible for the conduct of my team members during all league activities. This includes any damage caused to property, involvement in physical altercations, and the safety and well-being of the players in the league. I understand that the league is not liable for any injuries sustained by my players. 6. In consideration of having the opportunity to participate as a coach and/or volunteer, and in acknowledging that I an aware of and willing to assume the risks associated with this activity, I hereby voluntarily agree to waive, hold harmless and indemnify the THE COLLECTIVE, it’s affiliates and its trustees, agents, volunteers and employees from any and all claims, demands, damages and causes of action of any nature whatsoever arising out of ordinary negligence which I, my heirs, or my successors may have against them for, on account of, or by reason of my participation in the above activities. I indicate my agreement to this hold harmless elective noted below. TOURNAMENT RULES:

PLEASE ARRIVE TO ALL GAMES 15min before scheduled time to play.

ANY TEAM MORE THAN 10 minutes late will Forfeit their game .

Forfeit will lead to Disqualification from tournament and Future Events!

HIGHSCHOOL GAMES: will be 8 min quarters.

Clock will stop last 3 mins of the 4th quarter for each game.

Clock will not stop if a team is leading by 20 points or more

Each Team will receive 4 timeouts all together for the entire game.

OVERTIME WILL BE AN EXTRA 2 mins!

Elementary and middle school games will be 7 min quaters.

Overtime, and Forfeit rules apply to all Grades and Teams.

Only the head coach and assitant coach that is listed on this form can enter gym for free.

Coaches must have proof of age for all players on their rosters and it must be available to be presented if requested during the tournament.

$100 fee applicable when Challenging an opposing players age; If challenge is successful, you will retain said fee.
