Philadelphia Horse License Form
This form is for Philadelphia, PA residents who own horses, or persons who are using or offering the use of any non-commercial carriage horse pursuant to Section 10-100 of the Philadelphia Animal Code.

Horse licenses are issued by ACCT Philly only after the application has been reviewed and an onsite visit has been completed. Submitting this application does not guarantee that a horse license will be approved or issued.

If your application is approved, you will be sent a separate form to pay the $100 horse license fee.

Every license application must provide the name of the horse and identify the horse in one of the following ways:

(a) One or more color photographs of the horse sufficient to positively identify the horse;

(b) A specific written physical description of the horse sufficient to positively identify the horse by reference to breed, color, markings, and individual characteristics unique to the horse; or

(c) The identification number of a microchip permanently implanted in the horse, along with either a general written description of the horse's breed, color, and markings or a color photograph of the horse.
