Monthly Mentorship Questionnaire
This questionnaire is the next step to joining Monthly Mentorship. Your answers and required writing sample will help Katey and the guest faculty guide you to the next best step in your creative process.

Before proceeding with the Monthly Mentorship Questionnaire, we encourage all writers to do the following:
- Review The Breakdown section and FAQ at monthlymentorship.org to ensure you understand the weekly breakdown of the program, pricing, scholarship information, and the details about our Faculty's unique pedagogical approach.
- Review this entire questionnaire as well as the requirements for your writing sample prior to beginning to complete the application.
Some additional information about this application:
- This form will allow you to draft and save your responses prior to submitting. Please keep in mind that saving your responses is not the same as submitting the form.
- We highly recommend you draft your responses using this Google Document and copy and paste them into the form when you are ready to submit.
- Watch this video for an overview of what to expect when you submit your questionnaire. It can take a few seconds for the system to upload and attach your writing sample so do not close your browser window before receiving a "Thank you! Your submission has been received." message in your browser.

Your Writing Sample

Please review the following guidelines for submitting your writing sample carefully.
Feel free to select work that you feel is your strongest, and/or work that you feel stuck with and would therefore like to receive support around. The faculty will read your submission with care and be ready to speak with you about what resonates, where self-coaching might support your writing, and more.

Please upload a sample of your work following these guidelines:
- Be between 8 and 20 pages double-spaced in 12 pt. Times New Roman font with 1-inch margins on all sides.
  - You may submit more than one piece of writing, or writing in multiple genres, as long as you are within the page limit.
  - You may also offer a sentence or two of explanation at the top of your submission, if you'd like to do so (optional).
- The document can be in .doc, .docx, or PDF format and all pages should be in a single file.
- The first page of the document should include the title of your piece and the genre.
- Put your name and email address on the pages in case they get separated from your questionnaire.
- Also include your first and last name in the file name. Example: {name}_writing_sample.docx

Each year, WRITEABILITY, a a 501c-3 nonprofit founded by Katey, awards two Right to Write Awards given to two outstanding Monthly Mentorship applicants. The award supports BIPOC and Veteran writers by waiving their tuition (valued at $3650). Applicants must apply and be accepted to Monthly Mentorship before or in tandem with applying to the Right to Write Award. You can learn more here.
