PAYMENT AUTHORIZATION: By signing here, you agree to pay the dispatching fee to Rise Advantage LLC no later than each WEDNESDAY for the loads delivered during previous week (based on invoice generated no later than Friday every week).*

Limited Power of Attorney

This Limited Power of Attorney is made and entered into this date: Date* , by and between RISE ADVANTAGE LLC, a DELAWARE Company (“DISPATCH”), and Name of the company *, a Registered Motor Carrier with its principal office at Street Address * City * State* Zip Code*, (“CARRIER”); collectively referred to as the “Parties”.

The CARRIER authorizes RISE ADVANTAGE LLC to act as his trusted delegate and partner in front of the BROKERS, SHIPPERS, and other parties related to its transportation activity and therefore empowers the RISE ADVANTAGE LLC to:
A. Contact shippers and brokers on its behalf for any type of freight cargo;
B. Transfer of transportation documentation (Carrier Setup Packets, Rate Confirmations, Invoices, Bills of Lading, Lumper Receipts, Proofs of Delivery, etc.) to move any freight cargo for BROKERS/SHIPPERS;
C. Sign transportation Contracts (Setups) and Rate confirmations with freight BROKERS and direct SHIPPERS;
D. Transfer of legal CARRIERs documentation (W-9, FMCSA Authority Documents, Carrier Cargo and Liability Certificate of Insurance, SS-4 Form, and Carrier Company
Credentials, and other Documents) in the scope of accomplishing the object of the present contract;
E. Submitting load documents for the CARRIER (Bills of Lading, Proofs of Delivery, Rate Confirmations, Lumper Receipts, Invoices, etc.) to shippers, brokers, and/or current carrier factoring company for payment of delivered load, if requested;
F. Invoicing, Factoring Processing, Fuel Advance Processing, Accounts Receivables, and Collection Calls, if requested;
G. Request trip and oversize, overweight, and over-dimensional permits, if requested;
H. Any Processing requirements for the FMCSA (New Carrier Authority Setups and Filings, IFTA taxes, FORM 2290, Quarterly Filing, UCR, MC150, BOC3, IRP, Cab Card
Registration, Drug Consortium Review, Compliance.), if requested.

IN WITNESS WHEREOF, the parties hereto have executed this AGREEMENT on the date below and it shall remain in effect until terminated by either party.
