This form is the first step toward a bespoke, luxury experience designed entirely around you — your hair, your lifestyle, and your vision. Every detail you share helps me prepare for your consultation and ensures your service is perfectly customized from start to finish.

Before we begin, please take a moment to review a few important details:

Time to complete: This form will take approximately 10-15 minutes. Please answer thoughtfully — your honesty allows me to provide the safest, most beautiful, and seamless result possible.

Consultation & booking: Once your form is submitted (unless you’ve already scheduled), you’ll be contacted directly by me to schedule your consultation or full appointment . All consultations are $50, full appointments require $500 booking fee - payable at the time of booking and are non refundable . This booking fee will go towards your final total .

One guest per session : Each transformation is reserved exclusively for one guest per session . This ensures an unhurried, private, and truly personalized experience.

Complimentary refreshments: During your appointment, you’ll enjoy curated beverages, light snacks, and a catered lunch ( for appointments exceeding 4 hours ) — every detail is designed to make your visit relaxing and indulgent.

No gratuity: Gratuity is not expected - Your trust, satisfaction, and referrals are the greatest compliment I can receive.

Hair availability: Premium extension hair is kept in stock, allowing most guests to move forward without the need for a custom order.

Weft Extensions
Require in-salon maintenance every 5 -8 weeks. The hair itself is reusable, making this a great long-term investment with proper care.

K-Tip Extensions
These extensions are reusable . Meaning there will be down time between appointment removal & applications / having / 2 sets of hair is advised and will be talked about during consultation . You also have the option to have new hair each session 💕

Once you’ve reached the end of your wear time, new hair will need to be purchased or current hair will need to be re-tipped - This cost typically recurs every 3–4 months, depending on your hair density. Clients with finer hair may experience shorter wear times for safe, healthy results.

Extension Agreement Waiver
I acknowledge that my initial service is final after the application. Any changes to the style after application will
be charged accordingly and additionally.

I acknowledge that hair extensions are very sensitive and different from intact human hair. I have been informed of the daily maintenance procedure. I will follow the daily maintenance procedure to keep my
extensions in the best condition possible. I am fully aware and responsible that if I brush my hair hard or if the extensions are pulled that it can be ripped from the roots.

I understand my booking fee to secure an appointment with Nikaya - is non - refundable .
I have inspected and approved the hair extensions that are to be installed in my hair.
I understand I am paying for an overall total ‘look’—this means that any hair not used does not belong to me
unless discussed otherwise, and there is no price to break down further than simply a booking fee and remaining
total quote amount on install day.

In the event that I decide not to keep the hair extensions, I am fully responsible for the total payment of services rendered.

I also understand the explanations of the entire procedure, and I am aware that with proper care on my part,
Wefts should remain in my hair for no longer than 6-8 weeks between maintenance appointments.

I also understand ktips must be removed and reinstalled every 10-16 weeks depending on my natural hair density and rate of shed, which will be determined by my stylist.
I will come to my appointments with CLEAN, dry hair.

I will not go to sleep with wet hair, or without my hair braided or in a bun. I also understand a silk pillowcase & bonnet are strongly suggested!

I will follow the after care instructions given at all costs and I understand that any damage to my hair or the
extension hair will not be covered if not followed.
I understand that if an allergic reaction occurs, I will not hold my technician or salon at fault.
I have noted the charge for the removal of hair extensions is not included in the original fee.

I have read this waiver form in its entirety; I voluntarily accept the terms of this release by providing my
signature below, and warrant that I fully understand its contents.

Cancellation Policy:
I understand that things can come up. I value your time, and hope you value mine too. In the event you need to cancel your appointment, please be sure to notify me no later than 48 hours before your scheduled appointment. Failure to notify in time will incur a cancellation fee. To ensure our cancellation policy is upheld we require a credit card to book any appointment. Any appointment not cancelled within 48 hours of the scheduled date and time falls under my cancellation policy protocol. You will not be subject to any refunds of booking fees or any fees paid beforehand . If you have any questions regarding this policy please communicate them before committing to an appointment.

Late Policy:

At Nikaya Michelle Beauty Co., LLC | Hair By Nikaya | BOSS BEAUTY ®️ we do our best to accommodate everyone. Of course, life happens. Traffic, weather, accidents, etc. - we understand, if you are running late please call and make Nikaya aware. I want to be able to provide the desired service in its entirety. we will try our hardest to accommodate the services scheduled, however, after 15 minutes your appointment is subject to change depending on the stylists discretion. Please understand this is never a ‘punishment’ it is only to prevent overlap into the rest of the stylists appointments within the day. We thank you in advance for your consideration! *after 15 minutes past the scheduled appointment time, you will be considered no show and will not be subject to any refunds of any booking fees .

Saved Payment Method:
All guests' payment methods are safely and securely stored on file using my POS system. My POS system uses Square.

Service Adjustment Policy:

I understand if I would like something regarding my service that was spoken about in the consultation tweaked after leaving my scheduled appointment, Nikaya will be pleased to accommodate you free of charge in most circumstances. For said complimentary service to be done, this request must be made within 7 days of your initial appointment.

I understand no refunds are allowed under any circumstance.

Product Return Policy:

There are no returns exchanges on product sales.

Salon Etiquette:
Discriminatory/Derogatory statements and actions are prohibited in the salon and will not be tolerated. Our staff has the right to refuse service to anyone behaving improperly, intoxicated or if their state of health may influence the effects of the service.

Guest Policy:

The salon is not child and pet proof. hot styling tools, sharp scissors, chemicals and a busy salon can all become a safety hazard with children . Any Children or additional persons that are not actively receiving a service, are not permitted at the salon during your appointment time. Without prior written agreement Failure to follow this rule will result in your appointment being rescheduled and inability to receive any refunds of any kind due to inability to complete service .

Confidentiality and Privacy:
Every client has the right to privacy . Client information and email lists are confidential and will not be sold or shared with outside companies.

Consultation & Hair Condition Acknowledgment
I understand that to proceed with my consultation, I must arrive with my hair in its current state, length, and color, as these will be used to determine the best match for my extensions. Any changes in hair length or color between the consultation and the installation may result in a change in pricing or the cancellation of the appointment.

Media Release
I hereby grant Nikaya Warner AKA Hair By Nikaya permission to use my likeness in photographs, videos, or other digital media for promotional purposes, including web and social media publications. This primarily includes before-and-after photographs that may show my face. If I am not comfortable having my face shown on social media, only the back of my hair and the extension work may be published

1.) After submitting this form you will be contacted directly by Nikaya and will book a consultation .

2.) A $50 booking fee will be collected and your consultation appointment will be made . If you choose to book your full appointment a $500 booking fee will be collected and balance due will be due at the end of your completed service . Booking fees can be paid via Venmo, credit card over the phone , direct invoice or cash .

3.) At your consultation we will discuss your full hair plan / time needed / pricing/ aftercare etc . If you choose to book your appointment with Nikaya the $50 consultation fee will be applied to your service total .

4.) Attend your transformational appointment & balance due will be paid in cash to avoid credit card fees etc . Otherwise a 5% fee will be applied to all credit card/ Venmo etc charges .
