You, the participant, agree to assume all risks associated with any outcome resulting from your participation in this event. Possible side effects may include, but are not limited to: lightheadedness, mild fatigue, emotional release, physical tingling, body temperature change, and/or detox symptoms.

Sound Bath Healings are not suitable for everyone. People with pacemakers or defibrillator, any deep vein thrombosis in the legs, tumors, metallic implants, seizure disorder, or epilepsy are no advised to particpate in sound healing. If you have carotid arrhythmias, a stent, a shunt, deep brain stimulation device, or are pregnant, you should not participate without the consent of your healthcare professional.

In exchange for the fees, paid by you, Tammy Hulke and Gail Smith are providing a service assisting you in mind, body, spirit integration through the use of modalities that may include, but not limited to: Massage and Bodywork, Healing, Touch, Sound Bath-Healing, Guided Meditation, Chakra Balancing, Yoga, Spiritual Life and Health Coaching.

 

You, the participant, agree to assume all risks associated with any outcome resulting from your participation in group or private sessions, retreats and classes.

 

I understand my session does not take the place of professional medical or healthcare, which is 100% my responsibility. I will seek out a licensed physician or healthcare professional for any physical conditions or psychological concerns I have.

 

Any health advice given is ultimately, accepted by my own free will. I release any and all liabilities, including but not limited to legal, financial, or personal liability to Tammy Hulke and/or Gail Smith.

 

I understand and agree that Tammy Hulke and Gail Smith are not claiming the ability to heal my body or mind.

By signing this Waiver and Release you grant permission to the rights of your image, likeness and sound of your voice as recorded on audio or video for promotional use by Tammy Hulke and/or Gail Smith without payment or any other consideration. Photographic or video recordings of you may be electronically displayed via the internet or other promotional materials.

No refunds will be offered after August 5th.

If paying with PayPal you will be redirected to PayPal once you hit submit. There is a $5 processing fee for using this option.  

If paying with a check, please send the registration fee as soon as possible. Checks can be made out to Tammy Hulke and mailed to 2836 24th St SW Pine River, MN 56474. You can pay multiple registration fees with one check.  In memo, please put the attendee(s) name.
