• Advisor Meeting and Planning Session (AMPS) Registration Form

    This meeting will be held on November 10, 2025 from 10 AM to 3 PM at the World Golf Village Renaissance Resort in St. Augustine, FL. All SkillsUSA advisors and instructors are invited! Each attendee must complete the form below to register. There is no cost to participate, and lunch is provided. Please select the appropriate response below if you need to register for the Chapter Management Institute.

    The questions below are completely optional, but will assist in our planning for the meeting agenda and how we will best utilize our time. Your feedback is valuable, and we encourage you to answer as many or as few questions you'd like.
  • On the following pages, you will find the SkillsUSA Florida Code of Conduct, Personal Liability and Medical Release Form, and Inclusivity Statement. Participation in this event is voluntary; as such, participants must agree to all three forms in order to participate.

  • SkillsUSA Florida Code of Conduct

    This SkillsUSA Florida All-Hands Advisor Meeting is designed to be an educational function, and all plans are made with that objective in mind. SkillsUSA Florida wants every participant to have an enjoyable experience with careful attention paid to both safety and comfort. All conference participants are expected to conduct themselves in a manner that best represents SkillsUSA as the nation’s greatest career and technical education student organization.

    For everyone to receive the maximum benefits from participation, SkillsUSA Florida’s “Code of Conduct,” as established by its board of directors, must be followed at all times.

    Note that attendance is not mandatory. By voluntarily participating, you agree to follow the official conference rules and regulations or forfeit your personal rights to participate. 

    1. I will treat others with dignity and respect. Discrimination and bullying will not be tolerated under any circumstances. Behavior such as sharing racist images, making sexist comments or unwanted sexual advances, or physical violence of any kind will result in immediate dismissal from the conference, disqualification from competition, and permanent ban from future SkillsUSA Florida activities.

    2. I will act with integrity and not engage in cheating or plagiarism, submit work that is not my own, or prevent another member or contestant from acting with integrity.

    3. I will not encourage or facilitate the consumption of alcohol or illicit drugs, or abuse or prescription medication by any conference attendee or participant.

    4. I will report any incident or injury to SkillsUSA Florida staff, no matter how large or small.

    5. I will not participate in one-on-one counseling between a minor and an adult who is not their blood relative or legal guardian.

    6. I will not cause any damage to hotel rooms, property, or other items that do not belong to me.

    7. I will be courteous to other hotel guests, and will not run in the halls, slam doors, play in elevators, or otherwise disturb other guests on hotel property.

    8. I will not enter areas that are not publicly accessible.

  • SkillsUSA Personal Liability and Medical Release Form

    I hereby release Florida SkillsUSA Inc., its representatives, agents and employees from liability for any injury to the named person, resulting from any cause whatsoever occurring to the named person at any time while attending this Florida SkillsUSA Inc. event, including travel to and from the event, excepting only such injury or damage resulting from willful acts of Florida SkillsUSA Inc. representatives, agents or employees. I voluntarily assume all risk and danger relating to the event, whether occurring prior to, during or after the event.

    I voluntarily authorize the Florida SkillsUSA Inc. event medical services coordinator or designees to administer and/or obtain routine or emergency diagnostic procedures and/or routine or emergency medical treatment for the named person as deemed necessary in medical judgment. 

    I agree to indemnify and hold harmless Florida SkillsUSA Inc. and its medical services coordinator and/or and designees for any and all claims, demands, actions, rights of action, and/or judgments by or on behalf of the named person arising from medical procedures or treatment rendered in good faith and according to accepted medical standards.

    I understand that Florida SkillsUSA Inc. has implemented preventative health and safety measures at this event to help reduce the spread of COVID-19. I understand Florida SkillsUSA Inc. cannot guarantee that event attendees will not be exposed to or infected by COVID-19. As a conference participant, I acknowledge the contagious nature of COVID-19. By attending this event, I voluntarily assume the risk and responsibility for any possible exposure or infection.

    I have read and understand the SkillsUSA Florida Code of Conduct. I agree to follow all policies, procedures and practices as stated. I understand that this is an educational activity and I will apply myself for the purpose of learning at all times and uphold the finest qualities of SkillsUSA members.

    Florida SkillsUSA Inc. is not responsible or liable for any issues related to my participation in any in-person, hybrid or virtual SkillsUSA Florida event, including: technology issues or interruptions, malfunctions or failures; personal injury; illness; or damage to school property or individual property.

    By electronically signing this form, I am indicating I am over the age of 18 and agree to all of the above statements. All participants must submit this form to participate.

  • Inclusivity Statement and Expectations

    SkillsUSA Florida aims to be inclusive to all participants of SkillsUSA events to ensure a welcoming, safe, and respectful environment by reflecting the diverse interests of our membership. We remain committed to providing an inclusive environment for all participants, regardless of age, gender, gender expression or identity, race, ethnicity, nationality, health status, disability, relationship status, sexual orientation, socioeconomic status, faith, or religion. 

    For the purposes of this statement, “participant” includes student members, advisors, chaperones, business and industry partners, judges, volunteers, alumni, contractors, vendors, exhibitors, and anyone else present at a SkillsUSA Florida function or activity.

    EXPECTED BEHAVIOR
    All participants must adhere to the following expectations:
    ● Exercise consideration and respect in your speech and actions.
    ● Avoid demeaning, discriminatory, or harassing behavior or speech.
    ● Be mindful and respectful of your surroundings and fellow participants.
    ● Alert staff if you witness a violation, someone in distress, or a potentially dangerous situation.

    UNACCEPTABLE BEHAVIOR
    The list of unacceptable behaviors is outlined below. Participants must exercise common decency and professionalism in all interactions and behaviors. The list below is in no way exhaustive or inclusive.
    ● Intimidating, derogatory, or demeaning speech or actions by any participant.
    ● Displaying or otherwise distributing harmful or prejudicial verbal or written comments or visual images related to age, gender, gender expression or identity, race, ethnicity, nationality, health status, disability, relationship status, sexual orientation, socioeconomic status, faith, or religion.
    ● Use of nudity and/ or sexual images, obscene gestures or language.
    ● Deliberate stalking or following; harassing photography or recording; sustained disruption of talks or other events; real or implied threat of physical or professional damage or harm.
    ● Unwelcome and uninvited attention or contact or physical assault (including touching/ groping).
    ● Retaliation for reporting an incident or reporting an incident in bad faith.

    REPORTING UNACCEPTABLE BEHAVIOR
    If you feel uncomfortable or unwelcome as a result of another participant’s behavior, witness inappropriate behavior, or have concerns regarding another participant’s behavior, contact the closest SkillsUSA Florida staff member onsite or email jgraber@skillsusafl.org. If possible, provide any relevant details and documentation on this form: form.jotform.com/223326176041044
    ● Your name and contact information; date, time, and location of the incident.
    ● Identifying information of any offenders or victims (physical traits, contestant number, school, etc.)
    ● Description of the violating behavior, along with any physical injuries or property damage.
    ● Photos, videos, screenshots, or any other evidence; identifying information for any witnesses and any additional information that might be useful.

    Incidents will be investigated promptly, and advisors/ chaperones/ parents will be notified. SkillsUSA Florida has a zero-tolerance policy for unacceptable behavior and will take any necessary and warranted action.

    Potential consequences for violation:
    ● Point penalties in competition and/ or disqualification from competition.
    ● Removal from leadership positions, including State Officers and Courtesy Corps.
    ● Referral to school administration and/ or contact to parent/ guardian.
    ● Removal from the conference at own cost.
    ● Permanent ban from future SkillsUSA and SkillsUSA Florida activities.
    ● Legal action, including involvement of law enforcement agencies, to the fullest extent of the law.
