• Preparticipation Physical Evaluation Form that you need to take to your physician to be filled out
• Sports Physical Deferment Form is available for use for a limited time, but all athletes must have a physical completed by the end of the first month of each season — soccer (September), basketball and cheer (December), and baseball/softball (April).

• I hereby consent to have my son/daughter participate in interscholastic sports at Atlantic Christian School supervised by the teaching or coaching staff on or away from school grounds.
• I hereby authorize the staff member in charge to call an emergency ambulance in case of accident or acute illness, and to arrange for necessary emergency medical and surgical care, in case I am not immediately available. Any qualified physician called by the staff member may treat and do whatever is necessary for the health and well being of my son or daughter.
• It is understood that a conscientious effort must be made to notify me (parents) before such action will be taken. I also agree to accept responsibility for the cost of the above medical services.

Every athlete is required to have a sports physical on file before participating in competition. Forms must be completed by athletes and team managers participating in baseball, boys’ and girls’ basketball, cheerleading, golf, boys’ and girls’ soccer, softball, and track and field. The forms remain in effect for one year. There is also an Athletic Fee that must be paid for each sport in which a student participates, except for golf and track and field.

After you have completed the Physical Evaluation Form, please take a picture of each page and upload it using the fields in the following screens.

To participate on a school-sponsored interscholastic or intramural athletic team or squad, each student whose physical examination was completed more than 90 days prior to the first day of official practice shall provide a health history update questionnaire completed and signed by the student's parent or guardian.
